老年患者围术期高血压及其相关因素分析(1)

[摘要] 目的 观察老年患者在围术期高血压的发生情况及其影响高血压发生的相关因素。 方法 收集2008～2011年在我院进行手术患者15 320例，其中有3 910例为老年患者，对老年患者在围术期中的高血压发生情况进行比较分析。 结果 共有375例老年患者在围术期发生高血压，占9.59%；与老年患者围术期高血压发生情况有显著相关性的因素为患者是否在术前患有原发性高血压及其所实施的手术种类(P<0.05)；老年患者高血压发生情况在各种围手术期中差异具有统计学意义(P<0.05)，高血压在全麻各时段的发生率差异也有统计学意义(P<0.05)；在围术期发生共有86例有低血压的情况发生。 结论 与老年患者围术期高血压的发生情况有明显相关的因素为患者在术前是否患有原发性高血压及其患者所实施的手术种类；在围术期高血压患者容易出现低血压情况。

[Abstract] Objective To observe the elderly in the incidence of perioperative hypertension and its associated factors. Methods There were 15 320 case from 2008 to 2011 in our hospital for surgery, of which 3 910 cases were elderly patients, compared the incidence of hypertension of the elderly in the perioperative period. Results There were 375(9.59%) elderly patients in the perioperative hypertension appeared; The incidence of perioperative hypertension in the elderly patients was significantly related to whether the factors for patients with primary high preoperative blood pressure and the type of surgery (P<0.05); Incidence of hypertension in elderly patients in a variety of perioperative period had significant difference (P<0.05), high blood pressure in each period of anesthesia rate difference were also statistically significant(P<0.05); There were 86 patients had low blood pressure occurs. Conclusion Elderly patients with perioperative hypertension associated with primary hypertension before surgery and the type of surgery; Perioperative hypertension in patients prone to hypotension situation.

随着老年人数量的不断增多，我国面临着进入老龄化社会，随之而来的就是手术患者中老年患者所占的比例不断升高，这也显著增加了麻醉的危险性。在麻醉中较为常见的并发症之一就是围术期高血压，严重的高血压会使患者的危险性增大，尤其是对于老年患者，围术期高血压给其带来的高度危险性不容忽视[1]。本研究对近几年中来我院手术的患者进行观察分析，以期发现影响围术期高血压发生的相关因素，现报道如下。 ......
