早期机械通气配合亚低温对重型颅脑损伤疗效的影响(1)

     [摘要] 目的 探讨应用早期机械通气配合亚低温治疗重型颅脑损伤的疗效。 方法 2009年1月~2011年1月,将我科收治的50例重型颅脑损伤患者随机分为联合组(早期机械通气配合亚低温治疗)和对照组(早期机械通气治疗)各25例,比较两组患者血气变化情况及预后情况。 结果 观察组的PaO2、SaO2明显高于对照组,PaCO2明显低于对照组,差异有统计学意义(P<0.05);观察组的预后有效率明显高于对照组,差异有统计学意义(P<0.05)。 结论 早期机械通气配合亚低温对重型颅脑损伤具有较好的预后效果,能明显改善患者的血气指标,值得临床推广和应用。

    Therapeutic effect of early mechanical ventilation with mild hypothermia on severe head injury

    [Abstract] Objective To investigate the effect of early mechanical ventilation with mild hypothermia on severe head injury. Methods All of 50 patients with severe traumatic brain injury were randomly divided into the combined group (early hypothermia treatment with mechanical ventilation) and the control group (early mechanical ventilation) of the 25 cases from January 2009 to January 2011, and two groups were compared blood gas changes and prognosis. Results The combined group of PaO2, SaO2 were significantly higher than the control group, and PaCO2 was significantly lower than the control group, the difference was statistically significant (P<0.05). The prognosis of the observation group was significantly higher efficiency, and the difference was statistically significant (P<0.05). Conclusion Early mechanical ventilation with mild hypothermia in treatment of severe head injury has good prognosis, and can significantly improve the patient's blood gas analysis, should be widely promoted and applied.

    重型颅脑损伤后颅内压(ICP)增高,脑血流灌注不足及脑组织氧供需失衡,使患者出现较高的致死致残率。临床上亚低温对重度颅脑损伤后的脑保护作用已被证实,同时通过实施早期机械通气措施,能短暂降低ICP,改善脑灌注[1]。本文对2009年1月~2011年1月应用早期机械通气配合亚低温治疗重型颅脑损伤患者25例进行治疗,取得了较好的疗效 ......
