喉罩通气全麻用于乳腺癌根治术的临床观察(1)

     [摘要] 目的 探讨气管插管全麻和喉罩通气用于乳腺癌手术对血流动力学的影响。 方法 将60例乳腺癌手术患者随机分为两组,喉罩组(L组):全麻诱导后置入喉罩,插管组(M组):全麻后喉镜下气管插管维持通气,分别记录诱导前T0,置喉罩或气管插管前T1,喉罩置入或气管插管即刻T2、之后的3 min(T3)、5 min(T4)、15 min(T5)、拔除气管导管或喉罩时T6的平均动脉压(MAP)、心率(HR),记录术后24 h咽部疼痛及不适的发生率。 结果 喉罩通气组循环稳定(P < 0.05),术后的咽喉部不适发生率明显下降。 结论 喉罩通气应用于乳腺癌根治术是安全的,具有良好的血流动力学,明显降低由于气管插管造成的咽喉部不适。

    Clinical observation of application of laryngeal mask airway general anesthesia in radical surgery of breast cancer

    [Abstract] Objective To investigate the influence and the application of tracheal intubation general anesthesia and laryngeal mask airway in breast cancer surgery on hemodynamics. Methods Sixty patients receiving breast cancer surgery were randomly divided into two groups. For the laryngeal mask group (L group), the laryngeal mask was placed into the patients after the induction of general anesthesia. For the intubation group(M group), the laryngoscopic tracheal intubation under anesthesia was applied to maintain ventilation. The mean arterial pressures (MAP) and the heart rates (HR) at T0 before the induction, at T1 before the placement of laryngeal mask or tracheal intubation, at T2 at the placement of laryngeal mask or tracheal intubation, at T3 3 minutes, T4 5 minutes and T5 15 minutes after the placement of laryngeal mask or tracheal intubation, and at T6 at the removal of the tracheal tube or laryngeal mask were recorded. The incidence rates of pharyngeal pain and discomfort 24 hours after surgery were recorded. Results The laryngeal mask airway group had stabile circulation(P < 0.05) and significantly reduced incidence rate of postoperative throat discomfort. Conclusion The application of laryngeal mask airway in radical surgery of breast cancer is safe, has good hemodynamics and significantly reduces the pharyngeal discomfort caused by tracheal intubation ......
