新生儿缺氧缺血性脑病临床分度与CT征象相关性分析(1)

     [摘要] 目的 探討新生儿缺氧缺血性脑病(HIE)临床分度与CT征象间的相关性。 方法 自我院2013年9月~2017年2月已确诊的HIE新生患儿中选择200例为研究对象,给予CT检查和出生后Apgar评分,分析两者同HIE严重程度间的关联性。 结果 病灶集中分布于颞叶、额叶、白质内和枕叶等部位,CT征象显示存在脑水肿,在脑白质区域内集中分布有低密度灶,边界未见占位且模糊不清,同时发现脑室有部分狭窄性改变和完全闭塞,蛛网膜下腔、脑实质或脑室部位多伴有出血现象,但中线无移位;CT分度显示轻度HIE有78例(39.00%),与临床分度的108例(54.00%)有明显差异(P<0.01);CT、临床分度的中度及重度情况比较,无明显差异(P>0.05);轻度组的CT值、Apgar评分同中度组比较有显著差异(P<0.01),轻度组与重度组比较差异有统计学意义(P<0.05),中度与重度比较无显著差异(P>0.05)。 结论 诊断HIE时,应在临床病史及症状诊断的同时,使用CT检测,以提高诊断及预后判断的精确程度。

    Analysis on the correlation between clinical grading and CT Ssigns in neonatal hypoxic-ischemic encephalopathy

    [Abstract] Objective To explore the correlation between clinical grading and CT signs in neonatal hypoxic-ischemic encephalopathy(HIE). Methods 200 newborns who were diagnosed as neonatal HIE from April 2013 to February 2017 were selected as the study subjects in our hospital. CT and postnatal Apgar scores were given to them. The correlation between the two and the severity of HIE were analyzed. Results The lesions were mainly distributed in the temporal lobe, frontal lobe, white matter and occipital lobe. CT signs showed the presence of cerebral edema. In the white matter area, low-density lesions were distributed in concentration. The border was not occupied but blurred. It was also found that the ventricle showed some stenotic changes and complete occlusion. Subarachnoid, brain parenchyma or ventricular parts were associated with bleeding phenomenon,but the center line was without displacement; CT grading showed mild HIE in 78 patients(39.00%), which was significantly different from the 108 cases(54.00%) of the same clinical grading (P<0.01); the moderate and severe conditions of CT and clinical grading were compared, and there were no significant differences(P>0.05); there were significant differences in CT values and Apgar scores between mild group and moderate group (P<0.01), and there was significant difference between mild group and severe group(P<0.05). There was no significant difference between moderate group and severe group(P>0.05). Conclusion In the diagnosis of HIE, CT should be applied in the diagnosis of clinical history and symptom diagnosis, in order to improve the accuracy of determining diagnosis and prognosis., 百拇医药(许偈 邱丽霞 周盼盼)
