不同外科术式治疗复杂胫骨平台骨折的效果与生活质量对比(1)
[摘要] 目的 研究不同外科術式治疗复杂胫骨平台骨折的效果与生活质量。 方法 选取2016年10月～2018年9月我院和徐州矿务集团总医院收治的复杂性胫骨平台骨折患者58例，随机分成两组，对照组26例接受膝前正中直切口双钢板固定术，研究组32例接受膝内外双切口双钢板固定治疗，比较两组患者手术基本指标、膝关节功能和生活质量评分。 结果 研究组手术时间比对照组长，研究组住院天数、负重时间、骨折愈合时间短于对照组，差异有统计学意义(P<0.05)，但术中出血量方面，两组差异无统计学意义(P>0.05)。术后6个月随访时进行膝关节功能评价和生活质量评价，研究组患者HSS评分以及优良率皆优于对照组，差异有统计学意义(P<0.05);在生活质量评估方面，研究组患者术后6个月的SF-36评分高于对照组，差异有统计学意义(P<0.05)。 结论 膝内外双切口双钢板固定在治疗复杂胫骨平台骨折方面具有良好的临床疗效，可缩短患者住院时间，有助于骨折愈合以及膝关节功能的恢复，同时可改善患者术后生活质量。

[Abstract] Objective To study the effect and quality of life with different surgical treatments for complex tibial plateau fracture. Methods A total of 58 patients with complicated tibial plateau fracture admitted in our hospital from October 2016 to September 2018 were randomly divided into two groups. The control group(n=26) was treated with straight incision and double plate fixation in front of the knee. The study group(n=32) was treated with knee internal and external double incision double plate fixation. The basic parameters of surgery， knee function and quality of life score were compared between the two groups. Results The operation time of the study group was longer than that of the control group. The hospitalization days， weight-bearing time， and fracture healing time of the study group were shorter than those of the control group， and the difference was statistically significant(P<0.05). However， there was no statistical difference between the two groups in intraoperative blood loss(P>0.05). The knee function evaluation and quality of life evaluation were performed at postoperative 6-month follow-up. The HSS score and excellent rate of the study group were better than those of the control group， and the difference was statistically significant(P<0.05). In the quality of life assessment，the SF-36 score of the patients at 6 months after operation in the study group was higher than that of the control group， and the difference was statistically significant(P<0.05). Conclusion Knee internal and external double-incision double plate fixation has a good clinical effect in the treatment of complex tibial plateau fracture， which can shorten the length of hospital stay， contribute to fracture healing and recovery of knee function， and improve postoperative quality of life.
