无创双水平正压通气治疗早产儿呼吸暂停的临床研究(1)

     [摘要] 目的 研究无创双水平正压通气在早产儿呼吸暂停治疗中的应用。 方法 选取2017年8月~2019年6月本院新生儿科收治的77例呼吸暂停早产儿,将其随机分为对照组38例和实验组39例,对照组患儿采取CPAP治疗,实驗组患儿采取BiPAP治疗,观察并记录两组患儿治疗显效、有效、无效情况,检测并记录两组患儿血氧饱和度、动脉血氧分压、动脉血二氧化碳分压和酸碱度等血气分析指标并记录两组患儿并发症发生情况。 结果 实验组患儿治疗显效率(56.41%)、无效率(10.26%)和总有效率(89.74%)明显优于对照组(31.58%、31.58%、68.42%),差异具有统计学意义(P<0.05);实验组患儿治疗后血氧饱和度(0.92±0.10)、动脉血氧分压(81.75±18.45)mmHg、动脉血二氧化碳分压(35.68±13.83)mmHg和酸碱度(7.33±0.06)明显优于对照组[(0.85±0.09)、(72.38±20.16)mmHg、(48.61±11.52)mmHg、(7.29±0.08)],差异具有统计学意义(P<0.05);实验组患儿并发症发生率(12.82%),与对照组患儿(23.68%)相比,差异无统计学意义(P>0.05)。 结论 相比传统治疗方式,无创双水平正压通气能够有效治疗早产儿呼吸暂停,改善患儿各项血气指标,值得临床推广应用。

    [Abstract] Objective To study the application of noninvasive biphasic positive airway pressure ventilation(BiPAP) in the treatment of apnea in preterm infants. Methods 77 preterm infants with apnea admitted to our hospital from August 2017 to June 2019 were randomly divided into control group(n=38) and study group(n=39). The control group was treated with continuous positive airway pressure(CPAP), while the study group was treated with BiPAP. The significant effective, effective and ineffective rate, oxyhemoglobin saturation(SaO2), arterial partial pressure of oxygen(PaO2), partial pressure of carbon dioxide in artery(PaCO2), pH value and the incidence of complications were observed and compared between the two groups. Results The effective rate(56.41%), ineffective rate(10.26%) and the total effective rate(89.74%) in the study group were significantly better than those in the control group (31.58%, 31.58%, 68.42%), and the difference was statistically significant(P<0.05). The SaO2(0.92±0.10), PaO2 (81.75±18.45) mmHg, PaCO2 (35.68±13.83)mmHg and pH value(7.33±0.06) in the study group were significantly better than those in the control group [(0.85±0.09), (72.38±20.16) mmHg, (48.61±11.52) mmHg, (7.29±0.08)], and the differences were statistically significant(P<0.05). The incidence of complications in the study group(12.82%) was lower than that in the control group, and the difference was not statistically significant(P>0.05). Conclusion Compared with the traditional treatment, BiPAP can effectively treat the apnea of preterm infants, improve the blood gas indexes, which is worthy of clinical application.
