单乳管乳头溢液手术治疗分析

【摘要】目的 探讨乳头溢液的溢液特点、诊断方法和手术结果。方法 回顾性分析我院乳腺外科自2006年9月至2010年9月收治的65例单乳管乳头溢液患者的术前诊断、症状表现及术后结果的临床资料。结果 该组患者行区段切除术55例、改良根治术6例、单纯乳房切除术4例。术后行病理切片检查良性病变3例,恶性病变3例(其中乳头状瘤癌变2例,导管内癌1例),其余患者为一般乳腺疾病。结论 乳头溢液的病因复杂,根据具体情况酌情选择超声、X线、脱落细胞涂片、乳管造影、乳导管镜等可提高术前诊断率,影像学检查只可做出病灶的定位诊断,术中冰冻切片才是定性诊断的唯一方法。病变区乳腺的区段切除应成为大多数乳头溢液病人的手术方法,应该根据个体差异实施,尽量避免盲目地单纯乳腺切除。

【Abstract】 Objective To investigate the characteristic,diagnosis and treatment of the nipple discharge.Methods The clinical data of 65 cases who were diagnosed as nipple discharge september 2006 to september 2010 were collected and analyzed retrospectively。 Results 55 cascs used Section resection,6 cases used modified radical mastcetomy,4cases used sirnple mastectomy.Postoperative pathological examination did benign lesions 62 cases and m alignant lesions in 3 patients(Papilloma cancerous2 cases,intraductal carcinoma in 1 cases).Conclusion Theetiologyofnipple discharge is complex,according to the specific circumstances of ultrasonic,molybdenum palladium appropriate choice of X line,cells exfoliated smear,breast milk ducts stenosis,can increase preoperative diagnosis mtc ......
