血清肌钙蛋白T、心肌酶及白介素—6联合检测对慢性心衰患者的价值(1)
[摘要] 目的 探讨血清肌钙蛋白T、心肌酶及白介素-6联合检测对慢性心衰患者的价值。方法 随机选取2014年6月—2015年5月浙江大学医学院附属第一医院收治的67例慢性心衰患者作为观察组，62例体检健康者作为对照组。对比两组及不同程度慢性心衰患者的血清肌钙蛋白T、肌酸激酶、肌酸激酶同工酶及白介素-6水平。结果 观察组患者的血清肌钙蛋白T、肌酸激酶、肌酸激酶同工酶及白介素-6水平分别为(0.68±0.19) ng/mL、(118.76±23.52) U/L、(29.63±8.15)U/L及(16.81±3.56) ng/L，均明显高于对照组的(0.12±0.07) ng/mL、(56.97±16.82) U/L、(20.17±7.39)U/L及(6.85±1.68) ng/L(P<0.01)；心功能Ⅳ级患者的血清肌钙蛋白T、肌酸激酶、肌酸激酶同工酶及白介素-6水平分别为(0.75±0.25) ng/mL、(122.31±28.71) U/L、(33.46±10.38) U/L及(18.53±4.28) ng/L，均显著高于心功能Ⅲ级患者的(0.59±0.13) ng/mL、(99.63±18.19) U/L、(26.16±6.27 U/L及(15.78±2.15) ng/L(P<0.05)。 结论 慢性心衰患者的血清肌钙蛋白T、心肌酶及白介素-6水平均呈明显增高的趋势，它们的联合检测对心功能损伤程度具有较好的提示作用。

[Abstract] Objective To discuss the value of joint examination of serum cTnT， cardiac enzymes and IL-6 to patients with chronic heart failure. Methods In June 2014 - May 2015， 67 cases with chronic heart failure treated in the First Affiliated Hospital of Zhejiang University were observed as the observation group， 62 healthy persons were observed as the control group. The serum cTnT level， the CK level， the CK-MB level and the IL-6 level were compared between the two groups and the patients with varying degrees of chronic heart failure. Results The serum cTnT level， the CK level， the CK-MB level and the IL-6 level in the observation group patients respectively were (0.68±0.19)ng/mL， (118.76±23.52)U/L， (29.63±8.15)U/L and (16.81±3.56)ng/L， they were obviously higher than (0.12±0.07)ng/mL， (56.97±16.82)U/L， (20.17±7.39)U/L and (6.85±1.68)ng/L in the control group(P<0.01). The cTnT level， the CK level， the CK-MB level and the IL-6 level in the patients with heart function grade Ⅳ severally were (0.75±0.25)ng/mL， (122.31±28.71)U/L， (33.46±10.38)U/L and (18.53±4.28)ng/L， they were dramatically higher than (0.59±0.13)ng/mL， (99.63±18.19)U/L， (26.16±6.27)U/L and (15.78±2.15)ng/L of patients with heart function grade Ⅲ (P<0.05). Conclusion The serum cTnT level， the cardiac enzymes level and the IL-6 level of patients with chronic heart failure show a significant increasing trend. The joint examination of serum cTnT， cardiac enzymes and IL-6 has a good prompt action to the degree of heart function damage.

慢性心衰为心脏器质性或功能性疾病引起的进行性心室充盈和射血能力损害导致的一系列症状和体征[1]。该病虽无治愈可能，但在及时、有效的治疗措施下，不但能较好地控制症状，还能有效延长患者的生命，使预后效果得以提高[2]，因此，早期诊断具有重要意义。该研究分别对2014年6月—2015年5月浙江大学医学院附属第一医院收治的67例慢性心衰患者和62例体检正常者进行血清肌钙蛋白T(cardiac troponin T，cTnT)、心肌酶及白介素-6(interleukin-6，IL-6)的检测，并予以比较，旨在探讨其对慢性心衰患者的价值，有关情况，现报道如下。 (华继平)
