超声引导下穿刺冲洗治疗急性哺乳期乳腺脓肿对患者疼痛程度的影响(1)
[摘要] 目的 分析超声引导下穿刺冲刺治疗急性哺乳期乳腺脓肿对患者疼痛情况的影响。方法 方便选择2017年1—12月该院收治急性哺乳期乳腺脓肿患者156例，按照随机数字表法分成对照组(78例，切开引流术)和观察组(78例，超声引导下穿刺冲洗);比较两组患者疼痛程度[数字化评估表法(NRS)得分]、术后3个月患者对术后乳房外观的满意度。 结果 观察组NRS得分明显低于对照组(1～3分：78例 vs 1例;4～6分：0 例vs 37例;7～10分：0例 vs 40例)，差异有统计学意义(χ2=152.05、48.50、53.79，P<0.05);观察组患者满意度明显高于对照组(100.00% vs 37.18%)，差异有统计学意义(χ2=140.60，P<0.05)。 结论 超声下引导穿刺冲洗治疗急性哺乳期乳腺脓肿，患者疼痛感轻微，对术后乳房外观的满意度较高，临床推广价值高。

Ultrasound-guided Puncture Irrigation for Acute Breast-feeding Abscess on the Degree of Pain in Patients

[Abstract] Objective To analyze the effect of ultrasound-guided puncture and puncture on acute breast-feeding abscess in patients with pain. Methods 156 patients with breast abscess were convenient selected from January to December 2017 in the hospital. According to the random number table method， they were divided into control group (78 cases， incision and drainage) and observation group (78 cases， guided by ultrasound puncture washing); The degree of pain of the two groups of patients [NRS score] and the patient's appearance of postoperative breast appearance were compared at 3 months postoperatively. Results The NRS scores in the observation group were significantly lower than those in the control group (1 to 3 points： 78 cases vs 1 case; 4 to 6 points： 0 cases vs 37 cases; 7 to 10 points： 0 cases vs 40 cases). The difference was statistically significant. (χ2=152.05， 48.50， 53.79， P<0.05); The satisfaction degree of the observation group was significantly higher than that of the control group (100.00% vs 37.18%)， and the difference was statistically significant (χ2=140.60， P<0.05). Conclusion Ultrasound-guided puncture and irrigation is used to treat breast abscess in acute lactation. The pain of the patient is slight. It has a high degree of satisfaction with the appearance of the breast after surgery and has high clinical value.

急性哺乳期乳腺膿肿在初产妇中发病率较高，常在产后3个月内出现，主要是由急性乳腺炎症未能完全消散、吸收或者局限化所导致[1]。目前临床上对急性哺乳期乳腺脓肿的外科治疗以切开引流为主，但切开引流需通过肉芽填塞而愈合，患者愈合耗时长，且愈合过程中常需停止哺乳，患者愈合后乳房常遗留较大瘢痕，甚至出现因瘢痕收缩发生乳房变形等，不但没能消除患者生理上的疼痛，还可能对患者心理造化严重创伤[2]。随着超声技术的不断进步，微创手术由于其创口小，对患者伤害小等优点在临床上得到广泛的应用。因此，该研究将讨论该院2017年1—12月期间156例急性哺乳期乳腺脓肿分组分别进行超声引导下穿刺冲洗治疗和脓肿切开引流术治疗，比较两组患者疼痛程度，现报道如下。, 百拇医药(阿布力米提·阿布来提 阿布力克木·萨塔尔 艾散江·托合提)
