显微镜下精索静脉结扎术对精索静脉曲张患者的治疗效果及对精子活率的影响(1)
[摘要] 目的 探究顯微镜下精索静脉结扎术对精索静脉曲张患者的治疗效果以及对精子活率的影响。方法 选择该院2017年1月—2018年1月收治的精索静脉曲张(VC)患者作为研究对象，采用随机抽样法抽选出80例VC患者，将其按照入院时间顺序分为对照组、观察组(40例/组)。对照组VC患者行腹腔镜下高位结扎术，观察组VC患者开展显微镜下精索静脉结扎术，对比两组VC患者的治疗前后血清性激素水平、精液质量、并发症发生率等。 结果 观察组患者术后精子成活率为(55.67±9.73)%，显著高于治疗前(42.65±7.50)%(t=6.702 9，P<0.05)；与对照组相比，差异有统计学意义(t=2.135 8，P<0.05)。手术后6个月，对照组VC患者的性激素FSH、LH、T分别为：(5.62±1.32)mIU/mL、(3.87±1.54)mIU/mL、(3.41±0.91)μg/L，观察组患者的FSH、LH、T分别为：(4.24±1.45)mIU/mL、(3.12±1.38)mIU/mL、(3.90±1.10)μg/L，对照组与观察组相比差异有统计学意义(t=4.451 0、2.293 8、2.170 7，P<0.05)。 结论 与腹腔镜相比较，显微镜下精索静脉结扎术治疗效果更佳，患者术后并发症发生率较少，而且不易复发，精子质量得以提升，应用效果十分明显，值得推荐。

[Abstract] Objective To explore the therapeutic effect of spermatic vein ligation under the microscope on patients with varicocele and the effect on sperm viability. Methods The patients with varicocele (VC) treated in our hospital from January 2017 to January 2018 were selected as study objects. 80 patients with VC were selected by random sampling method， and they were divided into control group and observation group (40 cases/group) according to the time sequence of admission. VC patients in the control group were treated with laparoscopic high ligation， while VC patients in the observation group were treated with sphincter vein ligation under the microscope， and the serum sex hormone level， semen quality and complication rate before and after treatment were compared between the two groups. Results The survival rate of postoperative sperm in the observation group was (55.67±9.73)%， which was significantly higher than that before treatment (42.65±7.50)%(t=6.702 9，P<0.05)； compared with the control group， the difference was statistically significant(t=2.135 8 P<0.05). Six months after surgery， the sex hormones FSH， LH， and T of the control group were： (5.62±1.32)mIU/mL， (3.87±1.54)mIU/mL， and (3.41±0.91)μg/L， respectively. The FSH， LH， and T were (4.24±1.45)mIU/mL， (3.12±1.38)mIU/mL， and (3.90±1.10) μg/L， respectively.
