双柱锁定加压钢板与外固定支架在桡骨远端C型骨折治疗中的应用对比研究(1)
[摘要] 目的 对比在桡骨远端C型骨折治疗中应用双柱锁定加压钢板与外固定支架的实际价值。方法 2017年2月—2018年2月方便选取该院收治的70例桡骨远端C型骨折患者，结合随机表法将其列入观察组(采用双柱锁定加压钢板治疗)与对照组(采用外固定支架治疗)，观察两组患者的治疗效果。结果 观察组术后3个月、术后6个月的良好愈合率68.6%、88.6%高于对照组48.6%、72.1%，差异有统计学意义(χ2=12.547、14.084，P<0.05)，Gartland-Werly评分高于对照组，差异有统计学意义(t=5.218、5.198，P<0.05)，但两组术后9个月的Gartland-Werly评分以及良好愈合率(91.4% vs 88.6%)对比，差异无统计学意义(t=0.876，χ2=2.474，P>0.05);在骨性愈合时间、桡骨高度、尺偏角、掌倾角以及并发症情况方面，组间比较差异无统计学意义(t=-1.381、-0.409、-1.208、-2.714，P>0.05)。 结论 在桡骨远端C型骨折治疗中应用双柱锁定加压钢板的实际价值显著，短期疗效明显，安全性高，可予普及。

[Abstract] Objective To compare the practical value of double-column locking compression plate and external fixation in the treatment of C-type fracture of the distal radius. Methods From February 2017 to February 2018， 70 patients with type C fractures of the distal radius were convenient enrolled in our department. They were included in the observation group (treated with double-column locking compression plate) and the control group (external fixation was used) to compare the effect of the two groups of patients. Results The good healing rate of the observation group was 68.6% and 88.6% at 3 months and 6 months after operation， which was higher than that of the control group by 48.6% and 72.1%，the difference was statistically significant(χ2=12.547， 14.084， P<0.05). The Gartland-Werly score was higher than the control group，the difference was statistically significant(t=5.218， 5.198， P<0.05)， but the Gartland-Werly score and the good healing rate (91.4% vs 88.6%) at 9 months after surgery were not statistically significant (t=0.876， χ2=2.474， P>0.05); there was no statistically significant difference between the groups in terms of bone healing time， humeral height， ulnar angle， palm tilt angle and complications (t=-1.381， -0.409， -1.208， - 2.714， P>0.05). Conclusion The practical value of double-column locking compression plate in the treatment of C-type fracture of the distal radius is significant， short-term effect is obvious， safety is high， and it can be popularized.
