无创呼吸机辅助治疗慢阻肺合并Ⅱ型呼吸衰竭的 效果及有效率影响分析(1)

     [摘要] 目的 探討无创呼吸机辅助治疗慢阻肺合并Ⅱ型呼吸衰竭的临床效果。方法 方便选择2017年1月—2019年4月该院呼吸内科60例慢阻肺合并II型呼吸衰竭患者,按照随机数字表法分为两组,每组30例,两组均实施常规治疗,在此基础上,对照组采用经鼻导管低流量通气治疗,观察组采用无创呼吸机进行无创正压通气治疗,两组比较临床疗效、症状改善情况、肺通气功能指标、动脉血气分析指标。 结果 ①临床总有效率观察组96.67%高于对照组80.00%,差异有统计学意义(χ2=4.043,P=0.044)。②咳嗽、咳痰、呼吸困难等症状缓解时间观察组均短于对照组(t=4.280、4.621、4.184,P=0.000、0.000、0.000)。③治疗后,观察组的FEV1、FEV1/FVC与对照组相比均更高(t=4.130、4.310,P=0.000、0.000),观察组的PaO2高于对照组(t=3.995,P=0.000)。其PaCO2低于对照组(t=4.687,P=0.000)。结论 无创呼吸机辅助治疗慢阻肺合并II型呼吸衰竭的临床疗效显著,可更好地改善患者症状、肺通气功能及动脉血气状况。

    [Abstract] Objective To explore the clinical effect of non-invasive ventilator in the treatment of chronic obstructive pulmonary disease (COPD) with type II respiratory failure. Methods From January 2017 to April 2019, 60 cases of patients with chronic obstructive pulmonary disease complicated with type II respiratory failure in Respiratory Medicine Department of the hospital were conveniently selected and randomly divided into two groups according to random number table, 30 cases in each group. Both groups were given routine treatment, on this basis, the control group was given low flow ventilation through nasal catheter, while the observation group was given noninvasive positive pressure by noninvasive ventilator. The clinical efficacy, symptoms improvement, pulmonary ventilation function and arterial blood gas analysis were compared between the two groups. Results 1.The total effective rate of the observation group was 96.67% higher than that of the control group (80.00%), and the difference was statistically significant(χ2=4.043, P=0.044). 2. Cough, cough, dyspnea and other symptoms relief time were shorter in the observation group than in the control group(t=4.280, 4.621, 4.184, P=0.000, 0.000, 0.000).3. After treatment, the FEV1 and FEV1/FVC of the observation group were higher than the control group(t=4.130, 4.310, P=0.000, 0.000), and the PaO2 of the observation group was higher than that of the control group(t=3.995, P=0.000). Its PaCO2 was lower than the control group(t=4.687, P=0.000). Conclusion Non-invasive ventilator-assisted therapy for chronic obstructive pulmonary disease complicated with type II respiratory failure has a significant clinical effect, which can better improve the symptoms, pulmonary ventilation function and arterial blood gas status of patients.
