儿童眼眶占位性病变的分布与CT影像特点(1)

     [摘要] 目的 探讨儿童眼眶占位性病变的分布与CT影像表现。方法 该研究回顾性分析2016年3月—2019年2月住院部收治的40例经病理证实的眼眶占位性病变儿童患者的临床及CT影像资料。所有患者均行多排螺旋CT,记录眼眶占位性病变的分布及CT影像学特点,病变根据软组织的解剖结构,将眼眶分为4区,分析病灶直径及CT值。结果儿童眼眶占位性病变主要為皮样囊肿、表皮样囊肿,分为55.0%和37.5%,其他还包括多形性腺瘤、海绵状血管瘤、炎性假瘤。儿童患者CT影像分析病变多发生在骨膜外区(70%),肌锥外区、眼球筋膜囊区比例分别为15%、10%,未发现混合区患者。肌锥外区平均病灶直径最大,且CT值也最高,眼球筋膜囊区平均病灶直径最小,CT值也最低,病灶直径与CT值正相关。眼球筋膜囊区最大特点为CT值﹙-7.62±20.34﹚ HU。 结论 儿童眼眶占位性病变主要为皮样囊肿、表皮样囊肿且发生在骨膜外区,眼眶4区病灶直径与CT值正相关,眼球筋膜囊区特点为负CT值。

    [Abstract]ObjectiveTo investigate the distribution and CT imaging findings of children's orbital space-occupying lesions.MethodsThis study retrospectively analyzed the clinical and CT imaging data of 40 patients with pathologically confirmed orbital space-occupying lesions admitted to the inpatient department from March 2016 to February 2019. All patients underwent multi-slice spiral CT, recording the distribution of orbital space-occupying lesions and CT imaging features. The lesions were divided into 4 regions according to the anatomical structure of the soft tissue, and the lesion diameter and CT value were analyzed. Results The main orbital space-occupying lesions in adolescents were dermoid cyst and epidermoid cyst, which were divided into 55.0% and 37.5%. Other lesions included pleomorphic adenoma, cavernous hemangioma and inflammatory pseudotumor. CT image analysis of adolescent patients showed that most of the lesions occurred in the extraperiosteal area (70%). The proportion of extrapyramidal area and ocular fascial sac were 15% and 10% respectively. No mixed area was found. The average diameter of the lesion in the extrapyramidal area was the largest, and the CT value was the highest. The average diameter of the lesion in the ocular fascial sac area was the smallest, and the CT value was the lowest. The diameter of the lesion was positively correlated with the CT value. The most prominent feature of the ocular fascial sac area is the CT value ﹙-7.62±20.34﹚ HU. Conclusion The main orbital space-occupying lesions in adolescents are dermoid cysts and epidermoid cysts, which occur in the extraperiosteal area. The diameter of lesions in the orbital area 4 is positively correlated with CT value, and the feature of the orbital fascial sac area is negative CT value.
