短期胰岛素泵强化治疗对2型糖尿病患者胰岛素抵抗和血清瘦素水平的影响(1)

     [摘要] 目的:观察短期胰岛素泵(CSII)强化治疗对2型糖尿病(T2DM)患者胰岛素抵抗及血清瘦素(lep)水平的影响。方法:本院内分泌科住院初诊的T2DM患者52例采用CSII治疗。血糖控制持续2周后停止CSII。采用监测HOMA-IR胰岛素抵抗程度,血清lep测定采用BA-ELISA法。结果:CSII治疗2周后,患者的FPG明显下降,差异有统计学意义(t=25.948, P=0.000),达到治疗目标。而伴随着FPG的下降,患者的胰岛素抵抗水平也有改善,HOMA-IR下降(t=4.000, P=0.000)。同时血清lep水平治疗后也比治疗前有所下降(t=3.010, P=0.002)。HOMA-IR和血清lep水平呈正相关,相关系数r为0.577(t=4.583,P=0.014)。结论:短期胰岛素泵治疗2型糖尿病能有效降低血糖,并能改善患者的胰岛素抵抗,降低血清瘦素水平。

    Changes of insulin resistance and serum leptin concentration after short-term continuous subcutaneous insulin infusion therapy in type 2 diabetes

    [Abstract] Objective: To observe the changes of insulin resistance and serum leptin(lep) concentration after short-term continuous subcutaneous insulin infusion(CSII) treatment on type 2 diabetes(T2DM). Methods: We treated with CSII for 52 patients with T2DM in our hospital. CSII treatment were stopped at the time of 2 weeks after glycemic normal. The insulin resistance were assessed by HOMA-IR, serum lep were determined by BA-ELISA.Results: 2 weeks after CSII treatment, FPG were decreased(t=25.948, P=0.000), which achieved treatment goals. And the insulin resistance were also improved, it showed the HOMA-IR decreased(t=4.000, P=0.000). The concentration of serum lep were also decreased than before treatment(t=3.010, P=0.002). Correlation analysis showed that HOMA-IR had positive correlation with serum lep, the correlation coefficient was r=0.577(t=4.583, P=0.014). Conclusion: Short-term CSIItreatment for type 2 diabetes can reduce blood sugar, improve insulin resistance, decrease serum leptin concentration.
