老年慢性心力衰竭患者窦性心率震荡与心功能的相关性分析(1)

[摘要] 目的：旨在探讨CHF患者HRT与心功能相关性及其临床意义。方法：按纳入标准随机选择62例老年CHF患者作为观察组(心力衰竭组)，44例老年非心力衰竭心脏病患者作为对照组(非心力衰竭组)，均行超声心动图(UCG)、24 h动态心电图(Holter)检查，观察指标为TO、TS、LVEDD、LVEF等。结果：CHF患者TO、TS与LVEDD呈负相关，与LVEF呈正相关(P<0.05或P<0.001)。CHF患者LVEF越低，LVEDD越大，TS值越低。结论：HRT可作为评估CHF患者病情严重程度及其预后的有价值的无创指标。

[Abstract] Objectives: To study the correlation of heart rate turbulence (HRT) in elderly patients with chronic heart failure (CHF) and cardiac function and its clinical significance. Methods: According to the inclusion criteria and random principle, 62 elderly patients with CHF and 44 without heart failure of older patients with heart disease were enrolled and classified into two groups, research group (CHF group, n=62) and control group (non-CHF group, n=44). Two groups of patients accepted the doppler echocardiography (UCG), 24 hours ambulatory electrocardiogram (Holter) examination, observing indexes of TO, TS, LVEDD, and LVEF etc. Results: The TO and TS were negatively related to LVEDD and positively related to LVEF (P<0.05 or P<0.001) in CHF group. Conclusion: The HRT can be used as valuable noninvasive indices of evaluating severity and prognosis of heart failure.

慢性心力衰竭(chronic heart failure，CHF)患者是心脏性猝死的高危人群，对其进行危险分层及预后评估，进而采取干预性治疗，一直倍受临床关注。心率震荡(HRT)是一种检测和评估体内自主神经调节功能平衡及稳定性的新技术。目前将HRT的变化用于CHF患者的危险分层及预后评估的研究已成为热点。笔者通过对62例老年CHF患者与44例老年非心力衰竭心脏病患者HRT、心功能的分析，探讨了它们之间的相关性。现报道如下：
1 资料与方法
1.1 一般资料
随机选择2010年6～12月本院心内科老年CHF患者62例作为观察组(心力衰竭组)，其中按NYHA分级心功能Ⅱ级30例、Ⅲ级26例、Ⅳ级6例；女26例，男36例，平均年龄(70.68±7.62)岁；选择老年非心力衰竭心脏病患者44例作为对照组(非心力衰竭组)，其中，女16例，男28例，平均年龄(68.84±6 ......
