调强放射治疗与三维适形放射治疗疗效比较

     [摘要] 目的:比较调强放射治疗与三维适形放射治疗在肿瘤放射治疗中的疗效。方法:将42例需要放射治疗的肿瘤患者,随机分为调强放射治疗组24例和三维适形放射治疗组18例,分别采用不同的放射治疗方法给予放射治疗。结果:在宫颈癌的放射治疗中,使用三维适形放射治疗PTV内的最大剂量为63.6 Gy,直肠和膀胱的平均剂量分别是40.6 Gy和44.8 Gy;使用调强放射治疗PTV内的最大剂量是61.3 Gy,直肠和膀胱的平均剂量分别是22.8 Gy和25.9 Gy。在扁桃体癌的放射治疗中,使用三维适形放射治疗PTV内的最小剂量是39.8 Gy,脊髓的最小剂量是29.0 Gy;使用调强放射治疗PTV内的最小剂量是57.9 Gy;脊髓的最小剂量是12.8 Gy。在上肢软组织肉瘤术后治疗中,使用三维适形放射治疗脊髓最小剂量是45.0 Gy;使用调强放射治疗脊髓最小剂量是14.4 Gy。结论:调强放射治疗与三维适形放射治疗技术相比较,前者能使放射治疗的靶组织内的剂量分布均匀一致性得到明显的改善。四肢软组织肉瘤术后,不能采用三维适形放射治疗,但可以采用调强放射治疗。

    Comparison of radioactive treatment effect between intensity modulated radiotherapy (IMRT) and three-dimensional conformal radiotherapy (3D-CRT)

    [Abstract] Objective: To compare radioactive treatment effect of tumour between IMRT and 3D-CRT. Methods: 42 patients who suffer from cancer were randomly divided into two groups. One group had 24 patients who were treated by IMRT and one group had 18 patients who were treated by 3D-CRT. Results: Radiotherapy in cervical cancer, the use of 3D-CRT the maximum dose within the PTV was 63.6 Gy, the average dose of rectum and bladder were respectively 40.6 Gy and 44.8 Gy; the use of IMRT the maximum dose within the PTV was 61.3 Gy, the average dose of rectum and bladder were respectively 22.8 Gy and 25.9 Gy. Radiation therapy in tonsillae cancer, the use of 3D-CRT the minimum dose within the PTV was 39.8 Gy, the minimum dose of the spinal cord was 29.0 Gy; the use of IMRT the minimum dose within the PTV was 57.9 Gy; the minimum dose of the spinal cord was 12.8 Gy. After operation of soft tissue sarcoma of the upper extremity the use of 3D-CRT the minimum dose of the spinal cord was 45.0 Gy; the use of IMRT the minimum dose of the spinal cord was 14.4 Gy. Conclusion: To compare technology between IMRT and 3D-CRT, the uniform of dose distribution in PTV of IMRT is much better than that of 3D-CRT. The soft tissue sarcoma of limb after operation can't be treated by 3D-CRT, but it can be treated by IMRT.
