[摘要] 目的 探讨人工股骨头置换与动力髋内固定治疗老年股骨粗隆间骨折的临床疗效。 方法 将本院收治的60例老年股骨粗隆间骨折患者随机分为人工股骨头置换组(A组)和动力髋内固定组(B组)，每组各30例，比较两组患者的术中出血量、手术时间、下床负重时间、并发症发生情况及Harris评分优良率。 结果 A组患者的术中出血量、手术时间均较B组长，差异有统计学意义，P < 0.05；两组患者的Harris评分优良率情况无差异；A组患者下床负重时间明显短于B组，差异有统计学意义，P < 0.05。A组患者的并发症发生率为16.7%，明显低于B组的36.7%，P < 0.05。 结论 人工股骨头置换治疗老年股骨粗隆间骨折的下床负重时间、并发症发生率明显优于动力髋内固定，但两者的疗效无差异，应根据患者的具体情况选择合适的治疗方法。

[Abstract] Objective To explore the clinical curative effect of the treatment of femoral intertrochanteric fracture in elderly patients by hemi-hip prosthesis replacement and dynamic hip screw. Methods Sixty elderly cases of intertrochanteric fractures were randomly divided into hemi-hip prosthesis replacement group (group A) and dynamic hip screw group (group B), retrospectively, and each group of 30 cases, the peri-operative bleeding, operation time, bed load time, complications and Harris score was compared between the two groups.
