热敷与穴位按摩对初产妇乳房胀痛及泌乳的影响(1)
[摘要] 目的 观察热敷与穴位按摩对初产妇乳房胀痛及泌乳的影响。 方法 2011年3月～2012年3月，按随机数字表法将本院产科足月分娩的初产妇200例随机分为实验组、对照组，各100例。实验组进行乳房热敷和穴位按摩，3次/d，连续3 d；对照组按照产科常规进行母乳喂养。3 d后评价乳房胀痛程度，5 d后观察母乳喂养情况。观察两组的乳房硬度、乳房充盈度、乳腺管通畅程度。 结果 实验组的乳房胀痛程度明显小于对照组(P<0.05)；实验组的母乳喂养情况显著优于对照组(P<0.05)；实验组的乳房硬度明显小于对照组(P<0.05)；实验组的乳房充盈度显著高于对照组(P<0.05)；实验组的乳腺管通畅通畅程度明显高于对照组(P<0.05)。 结论 早期乳房热敷和穴位按摩可以有效疏通初产妇乳络，改善和促进血液循环，促进乳汁分泌，减轻初产妇乳房胀痛程度，降低其乳房硬度，提高其乳房充盈度、乳腺管畅通度和母乳喂养率。

[Abstract] Objective To observe the influence of hot compress and acupoint massage on distending pain and lactation of breast in primiparas. Methods From March 2011 to March 2012，two hundred primiparas with full-term labor in department of obstetrics of our hospital were randomly divided into experimental group and control group according to the random number table method，and there were 100 cases in each group.The experimental group was given hot compress and acupoint massage of breast，3 times a day for 3 days continuously while control group was given breast-feeding according to obstetric routine.After 3 days，the degree of distending pain of breast was evaluated，and after 5 days，breast-feeding was observed.Hardness of breast，filling degree of breast and the patency of mammary duct was observed respectively. Results The degree of distending pain of breast in experimental group was obviously smaller than that of control group (P<0.05)；breast-feeding in experimental group was obviously better than that of control group (P<0.05)； hardness of breast in experimental group was obviously smaller than that of control group (P<0.05)；filling degree of breast in experimental group was obviously higher than that of control group (P<0.05)；the patency of mammary duct in experimental group was obviously higher than that of control group (P<0.05). Conclusion Hot compress and acupoint massage of breast in early stage can effectively dredge the meridians of breast in primiparas，improve and promote blood circulation，promote milk secretion，and relieve distending pain of breast and decrease hardness of breast，improve filling degree of breast，patency of mammary duct and increase the breast-feeding rate in primiparas.

对于刚分娩的初产妇来说，乳房胀痛和乳汁分泌少是两个既相关又矛盾的问题，如何在促进乳汁分泌的同时，减少乳房胀痛，增强初产妇的母乳喂养信心，提高母乳喂养率，是一个值得深入研究的课题。近年来，护理人员尝试了多种减轻初产妇乳房胀痛、促进泌乳的方法，取得了一定的效果。文献报道，乳房热敷或者乳房穴位按摩可在一定程度上疏通乳腺管、促进泌乳、减轻乳房胀痛，但是乳房热敷联合乳房穴位按摩在减轻乳房胀痛和促进泌乳方面的研究较少。笔者在学习借鉴文献方法的基础上，采用乳房热敷与穴位按摩相结合的方法，在促进初产妇泌乳，减少乳房胀痛，提高母乳喂养率方面取得较好的效果。 (熊芳梅 唐荣妹 郑芸)
