锁定钢板治疗股骨远端合并髁间复杂性骨折的临床效果(1)
[摘要] 目的 研究锁定钢板治疗股骨远端合并髁间复杂性骨折的临床效果。 方法 随机选取2012年6月～2014年1月本院收治的股骨远端合并髁间复杂性骨折患者80例为研究对象。根据入院时间将患者分为观察组和对照组，各40例。观察组采用锁定钢板手术进行治疗，对照组采用传统钢板手术治疗。比较两组患者的治疗效果。 结果 观察组的手术时间为(62.46±1.67)min，短于对照组的(106.45±3.42)min；出血量为(225.21±7.02)ml，少于对照组的(346.42±2.46)ml；住院天数为(12.35±0.86)d，短于对照组的(19.16±1.23)d；骨折愈合时间为(61.47±1.14)d，短于对照组的(97.27±2.40)d，差异有统计学意义(P<0.05)。治疗7～24个月，观察组的优良率为90.00%，高于对照组的57.50%(P<0.05)。 结论 锁定钢板治疗股骨远端合并髁间骨折的临床效果显著，可减少出血量，缩短骨折愈合时间，值得广泛推广。

[Abstract] Objective To study the clinical effect of locking plate treating complex fracture of distal femur combined with intercondy. Methods 80 patients with complex fracture of distal femur combined with intercondy treated in our hospital from June 2012 to January 2014 were randomly selected as study objects，and they were divided into observation group (n=40) and control group (n=40) according to admission time.Observation group was treated with locking plate operation while control group was treated with traditional plate operation.Treatment effect between two groups was compared. Results Operation time in observation group [(62.46±1.67) min] was shorter than that of control group [(106.45±3.42) min]，bleeding amount [(225.21±7.02) ml] in observation group was less than that of control group [(346.42±2.46) ml]，hospital stay in observation group [(12.35±0.86) d] was shorter than that of control group [(19.16±1.23) d]，fracture healing time in observation group [(61.47±1.14) d] was lower than that of control group [(97.27±2.40) d]and there was a statistical difference between two groups (P<0.05).After 7-24 months treatment，good and excellent rate in observation group (90.00%) was higher than that of control group (57.50%) (P<0.05). Conclusion Locking plate treating complex fracture of distal femur combined with intercondy has a significant effect，and can reduce the bleeding amount and shorten fracture healing time.It is worthy of wide promotion.

高能量损伤导致复杂性骨折是股骨远端合并髁间骨折的主要原因，患者的关节面容易受到损害，其治疗较困难且常伴有较多并发症，在治疗过程中要尽可能地满足解剖复位、坚强固定和早期进行功能锻炼，可使患者对膝关节功能恢复较满意[1]。股骨远端骨折约占整个股骨骨折总数的4%，加上骨折的不稳定性，容易波及患者的关节面，且并发症较多，致残率较高，目前是最难治疗的骨折之一[2]，因为保守治疗的效果较差，目前大多主张积极的治疗方案[3～5]。患者进行股骨髁间骨折手术后，膝关节的功能恢复不但与患者骨折的类型和手术方法有关，而且在很大程度上也与患者术后的膝关节功能锻炼、伤口并发症的预防、下肢水肿和深静脉栓塞的预防等护理、康复措施有关。为此，选取本院股骨远端合并髁间复杂性骨折手术治疗的患者以探究锁定钢板对其的效果，现报道如下。 (罗庆明 郑旭辉 鲍明霞)
