无张力疝修补术与传统疝修补术治疗老年腹股沟疝的效果比较(1)
[摘要] 目的 比较无张力疝修补术与传统疝修补术治疗老年腹股沟疝的效果。 方法 选取本院2013年3月～2014年9月收治的250例老年腹沟股疝患者作为研究对象，根据治疗方式的不同将其者分成观察组(126例)和对照组(124例)。观察组给予无张力疝修补术，对照组给予传统疝修补术。比较两组的手术时间、术中出血量等手术各项指标情况，术后随访观察疝复发、异物感等指标，比较其术后并发症发生率。 结果 观察组的手术时间、下床活动时间、术后疼痛时间、病情持续时间、恢复工作时间、住院时间显著短于对照组，差异有统计学意义(P<0.05)。观察组的术中出血量及应用止痛药次数显著少于对照组，差异有统计学意义(P<0.05)。观察组的住院费用显著高于对照组，差异有统计学意义(P<0.05)。两组的术后早期疼痛发生率比较，差异无统计学意义(P>0.05)。观察组的疝复发、异物感、切口感染、尿潴留、阴囊血肿、阴囊萎缩、远期疼痛、早期及远期活动受限及术区麻木感发生率显著低于对照组，差异有统计学意义(P<0.05)。 结论 无张力疝修补术能够降低老年腹股沟疝患者的术后早期及远期不良反应发生率，值得临床推广应用。

Effect comparison of tension-free hernioplasty and traditional hernioplasty in the treatment of elderly patients with inguinal hernia

[Abstract] Objective To compare the effect of tension-free hernioplasty and traditional hernioplasty in the treatment of elderly patients with inguinal hernia. Methods 250 elderly patients with inguinal hernia from March 2013 to September 2014 in our hospital were selected and divided into the observation group (n=126) and the control group (n=124) according to different treatment methods.The observation group was treated with tension-free hemioplasty，the control group was treated with traditional hernioplasty.The situation of operation index liked operation time and blood loss during operation was compared between two groups.The index liked relapse hernia and foreign body sensation was followed-up after operation.The incidence rate of complication after operation was compared. Results The operation time，activity out of bed time，postoperative pain time，duration of illness，recovery time，hospitalization time in the observation group was shorter than that in the control group，with significant difference (P<0.05).The amount of blood loss and the number of the painkiller application in the observation group was less than that in the control group，with significant difference (P<0.05).The operation cost in the observation group was higher than that in the control group，with significant difference (P<0.05).There was no significant difference between two groups in early pain after operation(P>0.05).The incidence rate of relapse hernia，foreign body sensation，incision infection，urinary retention，scrotum hematoma，scrotum atrophy，long-term pain，early and long-term limitation of motion and postoperative numbness in the observation group was lower than that in the control group，with significant difference (P<0.05). Conclusion Tension-free hemioplasty can reduce the incidence of postoperative early and long-term adverse reaction in elderly patients with inguinal hernia，it is worthy of clinical promotion and application.
