CK5/6、p63、CD10在乳腺良恶性病变中的表达及意义(1)
[摘要] 目的 研究CK5/6、p63、CD10在乳腺良恶性病变中的表达及意义。 方法 收集濮阳市妇幼保健院2010年1月～2015年12月乳腺增生性病变242例，其中普通性导管增生(UDH)151例(UDH组)，非典型性导管增生(ADH)54例(ADH组)，导管原位癌(DCIS)37例(DCIS组)；DCIS伴微浸润26例(DCIS伴微浸润组)；非特殊类型浸润性癌83例(非特殊类型浸润癌组)。采用免疫组化Envision两步法，检测各组标本中CK5/6、p63、CD10的表达情况。 结果 CK5/6的表达在UDH组明显高于ADH组、DCIS组、DCIS伴微浸润组、非特殊类型浸润癌组，差异有统计学意义(P<0.05)；CK5/6的表达在ADH组与DCIS组、DCIS伴微浸润组、非特殊类型浸润癌组间比较，差异有统计学意义(P<0.05)。p63、CD10在UDH组、ADH组、DCIS组的基底膜连续阳性表达无差异，在DCIS伴微浸润组和非特殊类型浸润性癌组中不表达，在UDH组、ADH组、DCIS组与DCIS伴微浸润组、非特殊类型浸润癌组间比较，差异有统计学意义(P<0.05)。 结论 联合检测CK5/6、p63、CD10三种指标能很好地鉴别乳腺良恶性病变。

[Abstract] Objective To study the expression and significance of CK5/6，p63 and CD10 in breast benign and malignant lesions. Methods 242 patients with breast proliferative lesions in our hospital from January 2010 to December 2015 were selected，among whom 151 patients had UDH (UDH group)，54 patients ADH (ADH group) and 37 patients DCIS (DCIS group)；26 patients had microinvasion (DCIS accompanied with microinvasion group)；83 patients had non-special invasive cancer (non-special invasive cancer group).Immunohistochemical Envison two-step method was applied to test the expression of CK5/6，p63 and CD10 in the specimen in each group. Results The expression of CK5/6 in UDH group was significantly higher than that in ADH group，DCIS group，DCIS accompanied with microinvasion group and non-special invasive cancer group，the differences were statistically significant (P<0.05)；The expression differences of CK5/6 among ADH group and DCIS group，DCIS accompanied with microinvasion group，non-special invasive cancer group were statistically significant (P<0.05).Successive positive expressions of p63 and CD10 in basement membrane in UDH group，ADH group and DCIS group were not different，they were not expressed in DCIS accompanied with microinvasion group and non-special invasive cancer group，there were statistical differences among UDH group，ADH group，DCIS group and DCIS accompanied with microinvasion group，non-special invasive cancer group (P<0.05). Conclusion The combined test of the three indicators of CK5/6，p63 and CD10 is favorable in identifying breast benign and malignant lesions.
