早期肠内营养对缺血性脑卒中合并吞咽困难患者营养状况及结局的影响(1)

     [摘要]目的 探讨早期肠内营养对缺血性脑卒中合并吞咽困难患者营养状况及结局的影响。方法 回顾性分析2012年12月~2015年12月本院收治的缺血性脑卒中合并吞咽困难患者80例,按入院先后顺序将其分为两组,各40例。观察组实施早期肠内营养,对照组实施早期胃肠外营养,比较两组饮水恢复时间、经口进食恢复时间,干预后体重指数(BMI)、腰围、臀围、血脂代谢情况,干预后两组神经功能缺损评分(NFI评分)及生活质量评分(QOL评分)。结果 观察组饮水恢复时间为(15.8±1.1)d,短于对照组的(32.5±3.9)d(P<0.01);经口进食恢复时间为(28.9±6.1)d,短于对照组的(46.9±10.2)d(P<0.01);观察组患者干预后BMI为(24.5±1.0)kg/m2,腰围为(93.7±2.1)cm,臀围为(96.7±3.4)cm,均大于对照组的(22.3±1.1)kg/m2、(89.7±1.6)cm和(94.6±3.2)cm(P<0.01);观察组总胆固醇为(4.3±0.3)mmol/L,三酰甘油为(1.3±0.1)mmol/L,均高于对照组的(3.9±0.2)mmol/L和(1.2±0.1)mmol/L(P<0.01);干预后观察组NFI评分为(24.1±1.1)分,QOL评分为(11.2±0.6),分,均显著低于对照组的(39.5±2.4)分和(18.5±1.3)分(P<0.01)。结论 对缺血性脑卒中患者实施早期胃肠内营养,可改善患者吞咽功能、提高机体血脂含量、促进神经功能恢复、提高生活质量,具有重要价值。

    [Abstract]Objective To investigate the effect of early enteral nutrition with nutritional status and outcomes for ischemic stroke with dysphagia.Methods 80 cases of ischemic stroke patients with dysphagia in our hospital from December 2012 to December 2015 were divided into two groups according to the order of admission,each 40 cases.The observation group were implemented with early enteral nutrition,the control group were implemented with gastrointestinal nutrition.The drinking water recovery time,food mouth recovery time,BMI,waist circumference and hip circumference,lipid metabolism,NFI rates and life of quality rates (QOL rates) were compared.Results The drinking recovery time in the observation group was (15.8±1.1) days,it was shorter than (32.5±3.9) days of control group(P<0.01);the oral feeding recovery time in the observation group was (28.9±6.1) days,it was shorter than (46.9±10.2) days of control group(P<0.01);the BMI in the observation group was (24.5±1.0) kg/m2,waist circumference in the observation group was (93.7±2.1) cm,hip circumference in the observation group was (96.7±3.4) cm,they were higher than (22.3±1.1) kg/m2,(89.7±1.6) cm and (94.6±3.2) cm in the control group(P<0.01);the total cholesterol in the observation group was (4.3±0.3) mmol/L and triglycerides in the observation group was(1.3±0.1) mmol/L,they were higher than (3.9±0.2) mmol/L and (1.2±0.1) mmol/L of control group (P<0.01);after the intervention NFI score in the observation group was (24.1±1.1) points and QOL score was (11.2±0.6) points,they were significantly lower than (39.5±2.4) points and (18.5±1.3) points of control group(P<0.01).Conclusion For patients with ischemic stroke,early enteral nutrition can improve swallowing function,improve the body lipid content,promote recovery of neurological function,and improve life quality.It has important value.
