胃充盈法超声检查在社区胃癌筛查中的应用价值(1)
[摘要]目的 探讨胃充盈超声检查在社区胃癌筛查中的应用价值。方法 选取2013年5月～2015年5月在我院接受胃癌篩查的500例社区人员作为研究对象，所有入选人员均予以胃充盈超声检查，根据胃镜检查和病例诊断结果，分析胃充盈超声检查的结果。结果 ①胃充盈超声检查共查出阳性病例405例，阳性检出率为81%；胃镜检查及病理诊断法共查出阳性病例415例，阳性检出率为83%。两种检查方式，差异无统计学意义(P>0.05)。②胃充盈超声检查法准确性为71%，胃镜检查及其病理诊断法准确性为70.2%，两种检查方式，差异无统计学意义(P>0.05)。结论 将胃充盈超声检查方式用于胃癌筛查，具有准确度高的优势，尤其将其用于儿童和老年对胃镜检查不耐受的群体，其价值更为显著，可将其在诊断、治疗、随访、复诊的环节中广泛应用。

[Abstract]Objective To explore the value of gastric filling ultrasonography in the screening of gastric cancer in the community.Methods From May 2013 to May 2015，500 community people who received gastric cancer screening in our hospital were selected as the subjects.All the participants were examined by gastric filling ultrasound.According to the results of gastroscopy and case diagnosis，the results of gastric filling ultrasound were analyzed.Results ①405 positive cases were detected by gastric filling ultrasound，and the positive rate was 81%.A total of 415 positive cases were detected by gastroscopy and pathological diagnosis，and the positive rate was 83%.There was no significant difference between the two groups(P>0.05).The accuracy of gastric filling ultrasound was 71%，and the accuracy of gastroscopy and pathological diagnosis was 70.2%.There was no significant difference between groups(P>0.05).Conclusion The gastric ultrasonography examination in screening of gastric cancer，with a high degree of accuracy，especially applied to children and the elderly to gastroscopy intolerance group，its value is more significant，therefore，can be widely used in the diagnosis，treatment，follow-up and referral links.

随着人们生活水平的提高，饮食习惯也发生了较大的变化，随之而来的胃肠道疾病及胃癌的发生率也呈现出逐年升高的趋势，胃癌作为临床常见的一种恶性肿瘤，预后性相对较差，为患者及家属带来不利的影响，而早期诊断和治疗，对改善患者预后，具有重要的价值和作用[1]。在人们传统的观念中均认为，超声对胃癌的检查缺乏准确性。导致这一观念产生的主要原因，胃腔中存在的大量气体，对超声检查图像的清晰度产生严重干扰，由此导致诊断结果准确性缺失，因此，探究一种提高超声诊断图像清晰度的方式，成为医学研究中重点关注的话题[2]。近年来，随着相关研究的不断深入，部分学者在实验研究中指出，增强型胃超声学造影剂(OUCA)可有效促进诊断图像清晰度的提高，这也使得超声在胃癌中的诊断得以实现[3]。鉴于此，本文特就胃充盈超声检查在胃癌筛查中的应用价值展开探析，现报道如下。, 百拇医药(喻永明 郭艳)
