加强术中护理对全膝关节置换患者术后膝关节肿胀及疼痛的影响(1)
[摘要]目的 探讨加强术中护理干预对全膝关节置换(TKA)患者术后膝关节肿胀及疼痛的影响。方法 选取2018年1～12月我院收治的72例TKA患者作为研究对象，采用随机数字表法分为研究组(n=36)和对照组(n=36)。对照组给予常规护理干预，研究組在常规护理基础上加强术中护理干预，比较两组术后膝关节肿胀及疼痛情况。结果 研究组膝关节肿胀程度轻于对照组，差异有统计学意义(P<0.05);术后第1、3、5天，研究组视觉模拟量表(VAS)评分低于对照组，差异有统计学意义(P<0.05);术后研究组的生活质量评分高于对照组，差异有统计学意义(P<0.05)。结论 TKA患者采用加强术中护理干预，可有效减轻术后膝关节肿胀程度，降低疼痛度，明显改善患者生活质量，而提高患者满意度。

Effect of strengthening nursing during operation on swelling and pain of knee joint after total knee arthroplasty

[Abstract] Objective To investigate the effect of strengthening nursing intervention during operation on swelling and pain of knee joint after total knee arthroplasty (TKA). Methods A total of 72 patients undergoing TKA who were treated in our hospital from January to December 2018 were selected as the research objects. They were divided into study group (n=36) and control group (n=36) according to the random number table method. The control group was given routine nursing intervention， the study group was given strengthening nursing intervention during operation on the basis of routine nursing. The postoperative knee joint swelling and pain of the two groups were compared. Results The degree of swelling of the knee joint in the study group was lighter than those in the control group (P<0.05); the visual analogue scale (VAS) scores of the study on the 1st， 3rd， and 5th day after the operation group were lower than those of the control group， which was statistically significant (P<0.05). The quality of life scores of patients in the study group were higher than those in the control group after surgery， with statistically significant difference (P<0.05). Conclusion Streagthening nursing interventions during operation for patients undergoing TKA can effectively reduce the degree of swelling of the knee joint， reduce pain， significantly improve the quality of life of patients， and thus increase patient satisfaction.

膝关节疾病是临床常见病及多发病，特别是晚期骨性关节炎、类风湿关节炎及创伤性关节炎，常导致膝关节疼痛和活动受限，严重影响患者生存质量[1-2]。目前，全膝关节置换术(TKA)是治疗此类膝关节疾病常用方法，利用人工材料替换病变膝关节，达到缓解疼痛、改善关节功能目的，但手术操作有一定创伤性，术后并发症发生率较高，术后常伴不同程度肿胀及疼痛，给患者带来巨大痛苦，不利于患者快速康复，给予科学有效护理干预十分必要[3-4]。对患者护理贯穿治疗整个过程，主要包括术前护理、术中护理及术后护理，有研究者将三部分看作一个整体，称为围术期护理[5]。临床实践中，病房护士和手术室护士是不同科室人员，很难做到同一个组护士完成患者术前、术中及术后整个护理过程，三部分护理对患者术后恢复同等重要。本研究旨在探讨加强术中护理干预对TKA患者术后膝关节肿胀及疼痛的影响，现报道如下。, 百拇医药(冯银珍 黄素珍 余建华 杨植栋 李留夷)
