     【摘要】目的:研究肥胖青少年与正常青少年在血糖、血脂等方面的差异,为对青少年肥胖的发生、发展施行健康干预提供理论依据。方法:将汕头市随机抽取的400例青少年,将体重指数(BMI)≥25 kg/m2的青少年纳入肥胖组(300例),BMI为18.5~23.0 kg/m2的纳入健康组(100例),抽取静脉血检测两组的空腹血糖(FBG),空腹胰岛素(FINS)、甘油三酯(TG),总胆固醇(TC)、瘦素的水平,并计算胰岛素抵抗指数(HOMA-IR)。结果:肥胖组的FINS、TG、HOMA-IR以及瘦素水平均明显高于健康组(P<0.01),而两组FBG和TC差异无统计学意义(P>0.05)。结论:肥胖青少年存在胰岛素抵抗和瘦素抵抗,并且具有潜在的糖脂代谢异常的代谢综合征的改变,这可能是青少年2型糖尿病发病率升高的原因之一。

    【Abstract】Objective:To compare the differences in insulin and cholesterol level between obese and healthy adolescents in order to provide theoretical basis to implement the healthy interventions to the obese adolescent.Methods:300 obese adolescents with body mass index (BMI)≥25 kg/m2 and 100 healthy adolescents ( BMI 18.5~23.0 kg/m2) were chosen randomly from the students in the City of Shantou. The levels of FBG,FINS,TG,TC and serum leptin of these adolescents were measured and compared,and calculate the HOMA-IR.Results:The levels of FINS,TG,HOMA-IR and leptin in obese group are much higher than those in the healthy group(P<0.01),while the differences between the two groups in the levels of FBG and TC was not significant (P>0.05).Conclusion:It is suggested that there are insulin and leptin resistance,as well as potential disorders in glucose and lipid metabolism in the obese adolescents.Our study indicated that adolescent obesity would have something to do with the early occurrence of type 2 diabetes.

    青少年的肥胖率逐年增高,已成为严重的公共卫生问题[1]。我国总体上处于青少年肥胖的早期流行阶段,近二十年来,经济发达大城市的青少年超重和肥胖上升了4~6倍,比发达国家流行早期更加迅猛[2]。因此,笔者研究了所在市肥胖青少年与正常青少年在血糖、血脂等方面的差异,为对青少年肥胖的发生、发展施行健康干预提供理论依据。

    1资料与方法

    1.1一般资料在汕头市的中、小学学生中随机抽取的400例,将体重指数(BMI)≥25 kg/m2的青少年纳入肥胖组(300例),BMI为18.5~23.0 kg/m2的纳入健康组(100例)。其中肥胖组男性152例,女性148例,平均年龄(15.51±1.87)岁。健康组男性56例,女性44例 ......
