实时三维超声心动图评估扩张型心肌病患者心室收缩功能的价值(1)
【摘要】 目的：探讨实时三维超声心动图评估扩张型心肌病患者心室收缩功能的价值。方法：选取本院2014年3月-2015年5月收治的39例扩张型心肌病患者为研究组，同期健康体检者39例为对照组，均给予实时三维超声及二维超声检查，比较两组患者的心室功能参数。结果：研究组的左、右室每搏量与对照组比较，差异无统计学意义(P>0.05)；研究组的左、右室射血分数低于对照组，而舒张末容积、收缩末容积则高于对照组，两组比较差异均有统计学意义(P<0.001)。研究组的二维和三维条件下心室收缩功能各指标比较差异均有统计学意义(P<0.01)；二维和三维条件下测量的左、右室射血分数呈正相关性(r=0.65、0.67，P<0.01)。结论：实时三维超声心动图技术可准确评价扩张型心肌病患者的左右心室功能，值得临床推广应用。

【Abstract】 Objective： To explore the value of real-time three-dimensional echocardiography to assess ventricular systolic function in patients with dilated cardiomyopathy.Method： 39 patients with DCM were selected as the study group in our hospital from March 2014 to May 2015， at the same time 39 cases healthy check-up as the control group，they were given real-time three-dimensional ultrasound and two-dimensional ultrasound checked， the two groups of patients with ventricular function parameters were compared.Result： The patients with left and right ventricular stroke volume in study group compared with those of control group， there were no statistically significant differences (P>0.05)；the left and right ventricular ejection fraction in study group were lower than those of the control group， end-diastolic volume and end-contraction volume were higher than those of control group， the differences were statistically significant(P<0.001).The study group patients with ventricular systolic function under the condition of 2D and 3D each indexes were statistically significant differences(P<0.01)；under the condition of 2D and 3D measurement of left and right ventricular ejection fraction was positively correlation (r=0.65，P<0.01；r=0.67，P<0.01).Conclusion：Real-time three-dimensional echocardiography can accurately evaluate left and right ventricular function in patients with DCM， it is worthy of clinical popularization and application.

扩张型心肌病(dilated cardiomyopathy，DCM)的特征为单侧心室或双侧心室扩大，并伴心室收缩功能减退，伴或不伴充血性心力衰竭，是一种原因尚未明的原发性心肌疾病，起病缓慢，呈进行性加重，病死率较高，早期以心脏扩大最为多见，超声心动图对其诊断较为明确[1-3]。实时三维超声心动图(real-time three-dimensional echocardiogaphy，RT-3DE)是心脏超声领域内的一项重大的技术突破，其在检测心脏容积、观察心脏形态以及评价心功能等方面具有重要作用，可以快速清晰显示心房、心室壁以及各个腔的立体图像，可为瓣膜疾病及先天性心脏病的诊断和手术治疗快速提供重要信息，其不仅可以显示解剖平面的二维超声图像，还可以观察立体形态，可快速成像，可使医生快速了解心脏病变与病变的程度，在心脏疾病的诊断中具有重大的作用[4]。实时三维超声心动图实施方便、简单，具有无创的特点，易于被患者及其家属所接受。本研究主要探讨实时三维超声心动图在评估扩张型心肌病患者心室收缩功能中的价值，以供临床参考，报道如下。, 百拇医药(何真)
