后正中小切口非融合手术对胸腰段脊柱骨折的短期疗效(1)
【摘要】 目的：探讨后正中小切口非融合手术对胸腰段脊柱骨折的短期疗效。方法：回顾性分析2005年2月-2010年8月在本院接受治疗的34例胸腰段脊柱骨折患者的临床资料，按照手术方法分为对照组和观察组，每组各17例。观察组患者采用后正中小切口非融合手术进行治疗，对照组患者采用前路短节段内固定联合伤椎置钉的手术方法进行治疗。观察两组患者的手术时间、术中出血量、伤口愈合、角度丢失、脊椎评分及伤椎活动度等情况，分析短期治疗效果。结果：观察组的手术时间(52.28±15.72)min明显短于对照组的(70.12±18.63)min，术中出血量(80.12±10.98)mL明显少于对照组的(100.36±20.64)mL，差异均有统计学意义(P<0.05)。观察组患者的伤口愈合、伤椎Cobb角、椎体前缘高度恢复、脊椎JOA评分及伤椎活动度等指标均明显优于对照组，差异均有统计学意义(P<0.05)。结论：后正中小切口非融合手术应用于胸腰段脊柱骨折中具有较好的短期治疗效果，手术时间短，骨折创伤小，且手术能够保留脊柱运动单元的完好无损，近期效果显著，值得在临床上推广应用。

【Abstract】 Objective：To investigate the short-term effect on back median small incision non fusion surgery for thoracolumbar spine fracture.Method：The clinical data of 34 patients with thoracolumbar spine fracture in our hospital from February 2005 to August 2010 were retrospectively analyzed，and they were divided into the control group and the observation group according to the operation method，17 cases in each group.The observation group was given small incision non fusion surgery for treatment，the control group was given short segment anterior internal fixation combined with vertebral pedicle screw surgery method for treatment.The operation time，intraoperative bleeding volume，wound healing，angle loss，spine score and vertebral activity of the two groups were observed and the short-term treatment effects were analyzed.Result：The operation time of the observation group was (52.28±15.72)min，which was significantly shorter than (70.12±18.63)min of the control group，and the intraoperative bleeding volume of the observation group was (80.12±10.98)mL，which was significantly less than (100.36±20.64)mL of the control group，the differences were statistically significant(P<0.05).The wound healing，injured vertebral Cobb angle，fanterior flange height recovery，spinal JOA score and vertebral activity of the observation group were significantly better than those of the control group，the differences were statistically significant(P<0.05).Conclusion：It has good short-term treatment effect of back median small incision non fusion surgery for thoracolumbar spine fracture，the operation time is short，the fracture wound is small，and the operation can keep the spinal movement unit intact，the recent effect is remarkable，it is worthy of clinical application and promotion.
