血压昼夜节律变化对急性脑梗死患者预后的影响研究(1)

     【摘要】 目的:探討血压昼夜节律变化对急性脑梗死患者治疗效果的影响,并分析其与预后的相关性。方法:选取2014年1月-2016年12月于本院就诊的150例急性脑梗死患者作为观察对象,根据血压夜间下降率情况将患者分为杓型组、非杓型组及反杓型组,其中杓型组患者56例,非杓型组患者60例,反杓型组患者34例。比较三组治疗前后神经功能及生活质量变化情况,并分析所有患者夜间血压下降率与神经功能及生活质量的相关性。结果:在12周的治疗过程中,三组NIHSS评分均呈下降趋势,GQOLI-74评分均呈升高趋势。入组时及治疗后4周,三组间NIHSS评分及GQOLI-74评分比较差异均无统计学意义(P>0.05);治疗后8、12周,三组间NIHSS评分及GQOLI-74评分比较差异均有统计学意义(P<0.05),其中NIHSS评分以杓型组最低,反杓型最高,GQOLI-74评分以杓型组最高,反杓型最低。同时,血压夜间下降率与NIHSS评分变化率及GQOLI-74评分变化率均呈显著的正向直线相关性(P<0.05)。结论:血压昼夜节律变化可对急性脑梗死患者的预后造成严重的影响,其中以杓型血压患者预后较为理想,且血压变化率越高,其预后越理想。

    【Abstract】 Objective:To explore the influence of the changes of blood pressure circadian rhythm in patients with acute cerebral infarction and analyze its correlation with prognosis.Method:From January 2014 to December 2016,150 patients with acute cerebral infarction treated in our hospital were selected and divided into scoop type group(56 cases),non-dipper group(60 cases) and convex type group(34 cases) according to the situation of nighttime blood pressure drop rate,compared the nerve function and quality of life at prior-treatment and posttreatment of three groups,then analyzed the influence of nighttime blood pressure drop rate for patient’s neural function and quality of life.Result:After 12 weeks of treatment,NIHSS score of three groups were all downswing,GQOLI-74 score were all escalating trend.NIHSS score and GQOLI-74 score of three groups at access groups and treated 4 weeks had no statistically significant(P>0.05).At 8,12 weeks after the treatment,NIHSS score and GQOLI-74 score among three groups had significant statistic differences,the lowest NIHSS score was the scoop type group,the highest NIHSS score was the convex type group(P<0.05),the highest GQOLI-74 score was the scoop type group,the lowest GQOLI-74 score was the convex type group(P<0.05).We also found a positive correlation between the nighttime blood pressure drop rate and the score of NIHSS and GQOLI-74(P<0.05).Conclusion:Blood pressure circadian rhythm changes can seriously influence the prognosis of patients with acute cerebral infarction,the convex type group has best prognosis,the larger the change rate,the better the prognosis.
