PFNA对老年股骨粗隆间骨折患者的疗效影响(1)
【摘要】 目的：研究微創股骨近端髓内钉(PFNA)对老年股骨粗隆间骨折患者疗效的影响。方法：选取2016年5月-2017年6月本院收治的老年股骨粗隆间骨折患者112例。按照随机数字表法将其分为观察组和对照组，每组各56例。观察组采用PFNA固定治疗，对照组采用股骨髁上骨牵引治疗。比较两组术后疼痛程度(VAS评分)、手术时间、术中出血量、下地部分负重时间、治疗效果及术后并发症发生情况。结果：术后1周，两组VAS评分比较差异无统计学意义(P>0.05)；术后2～4周，观察组VAS评分均低于对照组，比较差异均有统计学意义(P<0.05)。观察组患者手术时间、术中出血量及下地部分负重时间均少于对照组，比较差异均有统计学意义(P<0.05)；观察组总优良率为92.86%，高于对照组的73.21%( 字2=7.669，P=0.006)；观察组并发症发生率为8.93%，低于对照组的39.29%( 字2=14.104，P=0.000)。结论：PFNA治疗老年股骨粗隆间骨折，可降低患者疼痛感及并发症发生率，提高疗效，值得推广。

【Abstract】 Objective：To explore the effect of minimally invasive proximal femoral nail antirotation(PFNA) on elderly patients with intertrochanteric fractures of femur.Method：A total of 112 elderly patients with intertrochanteric fracture of femur from May 2016 to June 2017 in our hospital were selected.According to the random number table method，they were divided into observation group and control group，56 cases in each group.The observation group was treated with PFNA fixation，and control group was treated with femoral supracondylar bone traction.The postoperative pain degree(VAS score)，operative time，intraoperative blood loss，partial weight loading time，therapeutic effect and postoperative complications of two groups were compared.Result：After operation 1 week，the VAS scores of two groups were compared，the difference was not statistically significant(P>0.05)，after operation 2-4 weeks， the VAS scores of observation group were lower than those of control group，the differences were statistically significant(P<0.05).The operative time，intraoperative blood loss，partial weight loading time in observation group were less than those of control group，the differences were statistically significant(P<0.05).The total excellent rate of observation group was 92.86%，which was higher than 73.21% of control group( 字2=7.669，P=0.006).The incidence of complications in observation group was 8.93%，which was lower than 39.29% of control group( 字2=14.104，P=0.000).Conclusion：The treatment of intertrochanteric fracture of femur by PFNA can reduce the incidence of pain and complications，improve the curative effect，which is worth popularizing.
