女性黄体期与卵泡期对靶控输注依托咪酯镇静效应的影响研究(1)

     【摘要】 目的:探討女性黄体期、卵泡期对患者行靶控输注(TCI)依托咪酯诱导,测定血浆靶控浓度,并进一步分析女性黄体酮水平对镇静效果的影响。方法:选取2015年7月-2017年7月于本院行甲状腺次全切除术的62例患者为分析对象,依据月经周期分为黄体期组与卵泡期组,每组31例。术前均予以黄体酮水平测定,给予靶控输注依托咪酯,设定起始浓度为150 ng/mL,在达到设定浓度6 min后逐次递增50 ng/mL至患者意识消失,且脑电双频谱指数(BIS)降低至50为止,记录患者入室后的BIS值为50、意识消失时依托咪酯血浆靶控浓度。结果:黄体期组术前黄体酮水平为(16.10±9.03)nmol/L,明显高于卵泡期组的(1.33±0.78)nmol/L,差异有统计学意义(t=4.332,P<0.05);两组各时间点,NIBP、HR、SpO2及BIS值比较,差异均无统计学意义(P>0.05);黄体期组在意识消失及BIS值为50时,依托咪酯血浆靶控浓度均明显少于卵泡期组,差异有统计学意义(P<0.05)。结论:与卵泡期比较,黄体期给予依托咪酯麻醉诱导剂量显著降低,这可能与患者黄体酮水平有一定关系。

    Effects of Female Luteal Phase and Follicular Phase on the Sedation Effect of Etomidate by Target Controlled Infusion/ZHENG Peina,HE Dinghui.//Medical Innovation of China,2018,15(12):132-134

    【Abstract】 Objective:To investigate the induction of Etomidate in female luteal phase and follicular phase by target controlled infusion(TCI),to determine the plasma target control concentration,and further analyze the effect of female progesterone level on sedation.Method:A total of 62 patients who underwent subtotal thyroidectomy from July 2015 to July 2017 in our hospital were selected as analysis subjects.According to menstrual cycle,they were divided into luteum phase group and follicular phase group,31 cases in each group.Before operation,progesterone level was measured,Etomidate was injected with target controlled infusion,the initial concentration was 150 ng/mL,and the initial concentration was gradually increased by 50 ng/mL after reaching 6 min until the patient’s consciousness disappeared,and the bispectral index was reduced to 50. The plasma target control concentration of Etomidate were recorded,when the BIS value of the patients after entering the room was 50 and the consciousness disappeared.Result:The level of progesterone in Luteal phase group was(16.10±9.03)nmol/L,which was significantly higher than(1.33±0.78)nmol/L in follicular phase group,the difference was statistically significant(t=4.332,P<0.05).At any time points,NIBP,HR,SpO2 and BIS value of two groups were compared,the differences were not statistically significant(P>0.05).In luteal phase group,when consciousness disappeared and BIS value was 50,Etomidate plasma target control concentration was significantly lower than that in follicular phase group,the difference was statistically significant(P<0.05).Conclusion:Compared with the follicular phase,the dose of Etomidate anesthetic induction in the luteal stage is significantly reduced,which may be related to the level of progesterone in the patient., 百拇医药(郑佩娜 贺定辉)
