肠腺癌组织中CK7、CK20、CEA、Ki—67表达水平的临床病理意义(1)

     【摘要】 目的:探討肠腺癌肿瘤组织中CK7、CK20、CEA、Ki-67表达水平与其临床病理特征的关系。方法:选取2015年1月-2017年3月于本院确诊并进行治疗的肠癌患者139例,均采用免疫组织化学染色Envision法检测其肠腺癌组织中CK7、CK20、CEA、Ki-67表达水平。结果:患者CK7、CK20、CEA表达阳性率分别为10.79%(15/139)、90.65%(126/139)、90.65%(126/139),Ki-67的增殖指数为(61±22)%;CK7的表达在不同部位及淋巴结转移间比较差异均有统计学意义(P<0.05);CK20的表达在不同部位、肿瘤大小、组织学类型及临床分期之间比较差异均有统计学意义(P<0.05);CEA的表达在不同组织学类型及淋巴结转移间比较差异均有统计学意义(P<0.05);Ki-67的表达在组织学类型、临床分期之间比较差异均有统计学意义(P<0.05)。结论:CK7、CK20表达情况有助于腺癌来源部位的判断;CK7、CEA阳性表达者,其淋巴结转移能力更强;CK20可能促进腺癌细胞分化,CEA则可能阻碍细胞分化;Ki-67的增殖活性与肿瘤的低分化程度及高临床分期相关。

    【Abstract】 Objective:To investigate the relationship between the expression level of CK7,CK20,CEA,Ki-67 in intestinal adenocarcinoma and its clinicopathological features.Method:A total of 139 patients with colorectal cancer diagnosed and treated in our hospital from January 2015 to March 2017 were selected.The expression levels of CK7,CK20,CEA and Ki-67 in intestinal adenocarcinoma were detected by immunohistochemical staining and Envision.Result:The positive rates of CK7,CK20 and CEA were 10.79%(15/139),90.65%(126/139) and 90.65%(126/139) respectively,and the proliferation index of Ki-67 was(61±22)%.The expression of CK7 in different locations and lymph node metastases were compared,the differences were statistically significant(P<0.05).The expression of CK20 in different locations,tumor size,histological type and clinical stage were compared,the differences were statistically significant(P<0.05).The expression of CEA in different histological types and lymph node metastasis were compared,the differences were statistically significant(P<0.05).The expression of Ki-67 in histological type and clinical stage were compared,the differences were statistically significant(P<0.05).Conclusion:The expression of CK7 and CK20 are helpful to the prediction of the origin of adenocarcinoma,and the positive expression of CK7 and CEA has stronger lymph node metastasis.CK20 may promote differentiation of adenocarcinoma cells,while CEA may block cell differentiation,and the proliferation activity of Ki-67 is related to the low degree of tumor differentiation and high clinical stage.
