四维彩超在孕中晚期筛查胎儿唇腭裂中的价值分析(1)

     【摘要】 目的：分析四维彩超在孕中晚期筛查胎儿唇腭裂中的价值。方法：收集2014年10月-2017年11月本院经二维超声检查后疑似唇腭裂畸形的胎儿71例作为观察对象，均给予四维彩超行二次诊断。比较二维超声与四维彩超的诊断准确率，及两种筛查方式对唇腭裂畸形程度评估的准确度。结果：71例唇腭裂畸形胎儿经引产或产后确诊为唇裂18例、腭裂9例、唇腭裂44例。四维彩超检查诊断总准确率98.59%明显高于二维超声66.20%(P<0.05)。四維彩超检查诊断误诊率0、漏诊率1.41%均明显低于二维超声检查的9.86%、23.94%(P<0.05)。四维彩超与二维超声检查对Ⅲ度唇裂、腭裂、唇腭裂的诊断准确率均达100%，但四维彩超检查对Ⅰ度、Ⅱ度唇裂、腭裂、唇腭裂的诊断准确率均分别明显高于二维超声检查(P<0.05)。结论：四维彩超在孕中晚期筛查胎儿唇腭裂中价值显著，可明显提高诊断准确率，并可有效判断唇腭裂畸形程度，对提升临床生育质量具有重大价值，值得临床上推广及应用。

    【Abstract】 Objective：To analyze the value of four-dimensional color Doppler ultrasonography in the screening of fetal cheilopalatognathus in the second and last trimester of pregnancy.Method：A total of 71 fetuses with fetal cheilopalatognathus after two-dimensional ultrasonography in our hospital from October 2014 to November 2017 were collected as observation objects.All patients were given secondary diagnosis of four-dimensional color Doppler ultrasonography.The diagnostic accuracy of two-dimensional ultrasonography and four-dimensional color Doppler ultrasonography was compared，and the accuracy of the degree of cheilopalatognathus by both screening methods was analyzed.Result：A total of 71 fetuses with cleft lip and palate were diagnosed as cleft lip 18 cases，cleft palate 9 cases，and cheilopalatognathus 44 cases by induction of labor or postpartum.The total diagnostic accuracy of the four-dimensional color Doppler ultrasonography 98.59% was significantly higher than that of the two-dimensional ultrasonography 66.20%(P<0.05).The misdiagnosis rate and the missed diagnosis rate of the four-dimensional color Doppler ultrasonography 0 and 1.41% were significantly lower than those of the two-dimensional ultrasonography 9.86% and 23.94%(P<0.05).The diagnostic accuracy of the four-dimensional color Doppler ultrasonography and two-dimensional ultrasonography for degree Ⅲ of cleft lip，cleft palate，and cheilopalatognathus was 100%.However，the diagnostic accuracy of the four-dimensional color Doppler ultrasonography in the degree Ⅰ，degree Ⅱ of cleft lip，cleft palate，and cheilopalatognathus was significantly higher than those of two-dimensional ultrasonography(P<0.05).Conclusion：Four-dimensional color Doppler ultrasonography has significant value in the screening of fetal cheilopalatognathus in the second and last trimester of pregnancy.It can significantly improve the diagnostic accuracy，and can effectively determine the degree of cheilopalatognathus.It is of great value in improving the quality of clinical reproduction.It is worthy of promotion and application in clinical practice., 百拇医药(李欣欣)
