喘息性支气管炎与小儿哮喘患者血清中IgE、LTs、MBP含量相关性分析(1)

     【摘要】 目的:分析喘息性支气管炎患儿血清中免疫球蛋白E(IgE)、髓鞘碱性蛋白(MBP)、白三烯(LTs)含量与哮喘的相关性。方法:选取2015年9月-2017年2月在本院住院的喘息性支气管炎患儿108例,均进行退热、止咳、平喘治疗,经1年随访,脱落8例,最终纳入100例患儿。根据最终是否发展为哮喘将其分为研究组33例(最终发展为哮喘)和对照组67例(治疗后未发展为哮喘)。检测两组治疗前后血清IgE、LTs、MBP含量;分析患儿血清LTs、MBP、IgE含量与哮喘的相关性。结果:治疗后,对照组血清IgE、LTs、MBP含量均低于治疗前,研究组血清IgE、LTs、MBP含量均高于治疗前,比较差异均有统计学意义(P<0.05);治疗前后,研究组血清IgE、LTs、MBP含量均高于对照组,比较差异均有统计学意义(P<0.05);单因素及多因素分析显示IgE、LTs、MBP均是哮喘发作的影响因素(P<0.05)。结论:通过IgE、LTs、MBP水平可以初步评价喘息性支气管炎患儿预后及发展为哮喘,为喘息性支气管炎及哮喘的治疗、预防、诊断及预后评价提供新的临床指标和研究基础。

    【Abstract】 Objective:To analyzed the correlation between serum IGE,LTs,MBP and asthma in children with asthmatic bronchitis. Method:A total of 108 children with asthmatic bronchitis admitted to our hospital from September 2015 to February 2017 were selected,all patients were treated with antipyretic,cough relieving and antiasthmatic treatment. After 1 year of follow-up,8 cases were dropped out,and 100 children were eventually enrolled.According to whether asthma was developed in the final stage,they were divided into 33 cases of study group(eventually developing asthma) and 67 cases in control group(did not develop asthma after treatment).The serum levels of IgE,LTs and MBP were measured before and after treatment in two groups,the correlation between serum LTs,MBP and IgE levels and asthma was analyzed. Result:After treatment,the serum levels of IgE,LTs and MBP in control group were all lower than those of before treatment,the serum levels of IgE,LTs and MBP in study group were all higher than those of before treatment,the differences were statistically significant(P<0.05). Before and after treatment,the serum levels of IgE,LTs and MBP in study group were higher than those of control group,the differences were statistically significant(P<0.05). Univariate and multivariate analysis showed that IgE,LTs and MBP were all risk factors for asthma attack(P<0.05). Conclusion:Through the IgE,LTs,MBP level can be a preliminary evaluation of the prognosis of children with asthmatic bronchitis and for the development of asthma,for asthmatic bronchitis and asthma treatment,prevention,diagnosis and prognosis evaluation provide the new clinical index and research base.

    喘息性支气管炎泛指有喘息表现的急性支气管感染,是一种特殊类型的支气管炎。喘息性支气管炎是小儿常见的呼吸道疾病,其症状与哮喘类似,以呼吸道感染症状为主,若未进行有效干预治疗,部分患者可能发展为儿童哮喘[1]。目前觀点认为喘息性支气管炎由多种病毒、细菌感染引起,其中部分最终发展为持续性哮喘[2],甚至有研究认为喘息性支气管炎与哮喘是同一种疾病[3],提示两者有相同或相似的发病机制。免疫球蛋白E(IgE)在机体的炎症反应和免疫调节过程中发挥重要作用,近年来在哮喘发病中的作用受到越来越多学者的关注[4]。髓鞘碱性蛋白(MBP)是中枢神经系统(CNS)髓鞘的主要蛋白质,位于髓鞘浆膜面,维持CNS髓鞘结构和功能的稳定,具有神经组织特异性[5]。白三烯(LTs)作为一种重要的促炎症介质,直接引起支气管肺炎平滑肌收缩,对中性粒细胞、嗜酸性细胞具有强大的趋化作用,引起呼吸道炎症[6]。因此,本研究对2015年9月-2017年2月在本院就诊的喘息性支气管炎患儿血清中LTs、MBP、IgE水平进行观察,分析其与哮喘之间的关系,以期为喘息性支气管炎患儿的治疗、预后评价及小儿哮喘的预防提供基础研究依据。现报道如下。, http://www.100md.com(温文捷 张健 张柱林)
