恒牙活髓切断术的治疗效果研究(1)
【摘要】 目的：探讨活髓切断术在龋源性露髓恒牙中的应用效果。方法：选取2015年1月-2016年12月在本院口腔科治疗的龋源性露髓恒牙患者60例作为研究对象，共76颗牙，根据不同的治疗方式将其分为活髓切断术组(n=40颗)和根管治疗术组(n=36颗)，各30例。比较两组治疗效果、术后不良反应发生情况及患者的满意度。结果：根管治疗术组治疗成功率为92.50%，活髓切断术组成功率为94.44%，两组治疗效果比较，差异无统计学意义(字2=0.117，P=0.733)。牙髓切断术组治疗后仅发生牙髓感染1颗，根管治疗术组合计9颗，两组不良反应发生情况比较，差异有统计学意义(字2=6.450，P=0.000)。活髓切断术组患者的满意度為93.33%，根管治疗术组为73.33%，活髓切断术组患者的满意度明显高于根管治疗术组，差异有统计学意义(字2=4.320，P=0.001)。结论：活髓切断术治疗龋源性露髓的恒牙效果显著，降低术后不良反应率，最大限度地保存健康牙髓组织，避免牙体组织预备，患者接受程度高，值得临床推广。

【Abstract】 Objective：To explore the clinical effect of pulpotomy on the cariously exposed permanent teeth.Method：A total of 76 teeth in 60 patients with cariously exposed permanent teeth treated from January 2015 to December 2016 in the Department of Stomatology our hospital were selected as the study objects.According to different treatment methods， they were divided into pulpotomy surgery group (n=40 teeth) and root canal therapy surgery (n=36 teeth)，30 cases in each group.The therapeutic effect，the incidence of postoperative adverse reactions and satisfaction of patients were compared between the two groups.Result：The success rate of the root canal therapy surgery group was 92.50% and the pulpotomy surgery group was 94.44%，there was no significant difference in the therapeutic effect between the two groups (字2=0.117，P=0.733).In the pulpotomy surgery group，there was only 1 case adverse reaction of dental pulp infection after treatment，and 9 cases were occurred in the root canal therapy surgery，the incidence of adverse reactions was compared between the two groups，the difference was statistically significant (字2=6.450，P=0.000).The satisfaction degree of the patients in the pulpotomy surgery group was 93.33% and the root canal therapy surgery group was 73.33%，the satisfaction degree of the pulpotomy surgery group was significantly higher than that of the root canal therapy surgery group，the difference was statistically significant (字2=4.320，P=0.001).Conclusions：The effect of pulpotomy on the cariously exposed permanent teeth is significant.It can reduce postoperative adverse reaction，preserve healthy pulp tissue to the maximum extent，avoid the preparation of dental tissues.The patient’s acceptance is high，it is worth of clinical promotion。
