CAD/CAM全瓷嵌体修复后牙邻面牙体缺损的临床效果评价(1)

     【摘要】 目的:探究在后牙邻面牙体缺损的修复中应用CAD/CAM全瓷嵌体的临床效果。方法:选取2016年8月-2017年3月于本院接受后牙邻面牙体缺损修复的患者90例作为研究对象,共110颗牙,按照随机数字表法将其分为治疗组(n=50颗)和对照组(n=60颗),各45例。治疗组使用CAD/CAM全瓷嵌体,对照组使用复合树脂,比较两组的修复成功、边缘密合度成功和颜色匹配以及继发龋齿发生情况。结果:治疗组的修复成功率为98.00%,明显高于对照组的81.67%,差异有统计学意义(字2=7.49,P=0.01)。两组的边缘密合度成功和颜色匹配情况比较,差异均无统计学意义(P>0.05)。两组继发龋齿发生情况比较,差异无统计学意义(字2=0.92,P=0.34)。结论:在后牙邻面牙体缺损的修复中应用CAD/CAM全瓷嵌体可以改善修复效果,且实用性更强,可将这一修复材料进行推广。

    【Abstract】 Objective:To explore the clinical effect of CAD/CAM all ceramic inlay in the repair of approximal surface dental defect of posterior teeth.Method:A total of 90 patients with posterior approximal surface dental defect repair and 110 tooth admitted from August 2016 to March 2017 in our hospital were selected as the study objects,according to the random number table method,they were divided into treatment group (n=50 tooth)and control group (n=60 tooth),45 cases in each group.The treatment group was used CAD/CAM all ceramic inlay,the control group was used composite resin.The success of repair,the success of edge consistency,color matching and the incidence of secondary caries were compared between the two groups.Result:The success rate of repair in the treatment group was 98.00%,which was significantly higher than 81.67% in the control group,the difference was statistically significant (字2=7.49,P=0.01).There were no significant differences in the success of edge consistency and color matching between the two groups (P>0.05). There was no significant difference in the incidence of secondary caries between the two groups (字2=0.92,P=0.34).Conclusion:The application of CAD/CAM all ceramic inlay can improve the effect of restoration in the repair of Posterior approximal surface dental defect and is more practical,which can promote this repair material.

    牙鄰面牙体缺损是临床上较为常见的一种口腔疾病,后牙邻面龋坏是导致这一疾病发生的主要原因[1]。一旦牙邻面牙体缺损发生,患者在日常生活中就有可能发生食物嵌塞等问题,有时还会出现牙龈乳头发炎等情况,致使牙龈出血、疼痛,这些问题均会对患者的生存质量造成极大的影响[2]。传统的修复材料为复合树脂或银汞合金,虽然可以在短期内对牙体进行有效的修复,但是后期极有可能发生填充物脱落等问题,继而导致微渗透、继发龋齿、食物嵌塞、邻面悬突等问题的发生[3]。另外在治疗的过程中,银汞充填为了达到固位的目的,需要对邻面洞型予以扩展处理,直至面,最终使患者的牙体形成临洞型,会对健康的牙体组织造成影响,且与生物学原则相悖[4]。复合树脂虽然不需要扩展,但是也会出现固位不良、填充物脱落等问题,且在聚合收缩作用的影响下,牙体可能会出现微渗漏等问题[5]。CAD/CAM全瓷嵌体修复即计算机辅助设计/计算机辅助制造修复是一种创新性的修复方法,在国际上受到了很大的关注。本文将选取2016年8月-2017年3月于本院接受后牙邻面牙体缺损修复的患者90例作为研究对象,研究CAD/CAM全瓷嵌体在修复中的应用效果,现报道如下。, http://www.100md.com(刘钰宇)
