院内一体化急救护理流程对4.5h内静脉溶栓治疗急性缺血性脑卒中患者的影响(1)

     【摘要】 目的：探討院内一体化急救护理流程对4.5 h内静脉溶栓治疗急性缺血性脑卒中患者的影响。方法：将2017年6月-2018年5月本院收治的4.5 h内行静脉溶栓治疗的急性缺血性脑卒中患者41例设为研究组，另将2016年6月-2017年5月本院收治的4.5 h内行静脉溶栓治疗的急性缺血性脑卒中患者41例设为对照组。研究组按照院内一体化急救护理流程进行护理，对照组按照常规护理流程进行护理，比较两组溶栓等待和住院时间、存活率、治疗后7 d的NIHSS评分及不良反应发生情况。结果：研究组溶栓等待和住院时间、治疗后7 d的NIHSS评分均明显低于对照组，存活率明显高于对照组，差异均有统计学意义(P<0.05)。研究组不良反应发生率为12.20%明显低于对照组的31.71%，差异有统计学意义(字2=4.556，P=0.033)。结论：将院内一体化急救护理流程应用于4.5 h内静脉溶栓治疗急性缺血性脑卒中患者临床效果显著，可有效缩短溶栓等待和住院时间，降低不良反应发生率，提高患者存活率，促进神经功能恢复，值得临床推广应用。

    【Abstract】 Objective：To investigate the effect of in-hospital integrated emergency care procedure on patients with acute ischemic stroke treated by intravenous thrombolysis within 4.5 hours.Method：A total of 41 patients with acute ischemic stroke who received intravenous thrombolysis within 4.5 hours from June 2017 to May 2018 were set up as the study group and 41 patients with acute ischemic stroke who received intravenous thrombolysis within 4.5 hours from June 2016 to May 2017 were set up as the control group.The study group was nursed according to the in-hospital integrated emergency care procedure，the control group was nursed according to the routine care process.The thrombolytic waiting and hospitalization time，survival rate，NIHSS score after treatment 7 days and adverse reactions between the two groups were compared.Result：The thrombolytic waiting and hospitalization time and NIHSS score after treatment 7 days in the study group were significantly lower than those in the control group，the survival rate was significantly higher than that in the control group，the differences were statistically significant (P<0.05).The incidence of adverse reactions in the study group was 12.20%，which was significantly lower than 31.71% in the control group，the difference was statistically significant (字2=4.556，P=0.033).Conclusion：The application of in-hospital integrated emergency care procedure for patients with acute ischemic stroke treated by intravenous thrombolysis within 4.5 hours has significant clinical effect.It can effectively shorten the thrombolysis waiting and hospitalization time，reduce the incidence of adverse reactions，improve the survival rate of patients，promote the recovery of neurological function and is worthy of clinical application.
