超声弹性成像对甲状腺微小结节良恶性诊断的应用价值体会(1)

     【摘要】 目的:探讨超声弹性成像对甲状腺微小结节良恶性诊断的应用价值。方法:选取2018年4月-2019年4月本院收治的56例甲状腺微小结节患者为对象,均予以常规超声与超声弹性成像检查,分析两种方法对甲状腺微小结节的良恶性诊断情况。结果:超声弹性成像与病理学诊断符合率(98.12%)高于常规超声检查(85.71%),差异有统计学意义(P<0.05);超声弹性成像对良性甲状腺微小结节的鉴别诊断的准确率(97.37%)、特异度(75.00%)及敏感度(100%)高于超声检查(89.47%、60.00%、93.94%),差异均有统计学意义(P<0.05);超声弹性成像对恶性甲状腺微小结节的鉴别诊断的准确率(96.00%)、特异度(75.00%)及敏感度(100%)高于超声检查(80.00%、33.00%、86.36%),差异均有统计学意义(P<0.05)。结论:超声弹性成像进行良恶性甲状腺微小结节鉴别诊断的准确率、特异度、敏感度均较高,值得选用。

    [Abstract] Objective: To explore the value of ultrasound elastography for the diagnosis of benign and malignant thyroid micronodules. Method: A total of 56 patients with thyroid microtubules were admitted to our hospital from April 2018 to April 2019 as the objects. All patients were underwent routine ultrasound and ultrasound elastography. The benign and malignant diagnosis of thyroid micronodules by two methods were analyzed. Result: The consistent with pathological diagnosis rate of ultrasound elastography (98.12%) was higher than that of conventional ultrasound (85.71%), the difference was statistically significant (P<0.05). The accuracy (97.37%), specificity (75.00%) and sensitivity (100%) of ultrasound elastography in the differential diagnosis of benign thyroid nodules were higher than those of ultrasound examination (89.47%, 60.00% and 93.94%), the differences were statistically significant (P<0.05). The accuracy (96.00%), specificity (75.00%) and sensitivity (100%) of ultrasound elastography in the differential diagnosis of malignant thyroid nodules were higher than that of ultrasound examination (80.00%, 33.00% and 86.36%), and the differences were statistically significant (P<0.05). Conclusion: The benign and malignant thyroid microtubules have high accuracy, specificity and sensitivity through ultrasound elastography, which is worthy of selection.

    甲狀腺结节是临床常见甲状腺疾病的一种,根据其疾病性质可分作良性、恶性两种,其中良性患者需通过超声展开动态观察,以判断疾病发作情况,再指导治疗工作的展开,而恶性甲状腺结节患者则需及时通过手术进行病变组织切除,以防止疾病持续恶化[1]。然而,甲状腺微小结节病灶表现出隐匿性特征,不仅无明显特异性症状,而且病理结构复杂,良性、恶性结节的鉴别工作难度较大。现阶段,临床上多通过超声技术对甲状腺微小结节进行鉴别诊断,常规超声检查可对结节形态、血流信息等进行判断,但难以判断结节硬度特征,而结节硬度特征是恶性甲状腺微小结节的主要特征之一[2-3]。近年来,超声弹性成像技术逐渐被应用于甲状腺微小结节定性诊断中,通过对感兴趣目标进行动态/静态/准静态实施微小激励获取弹性信号,在准确判断结节组织硬度的基础上,可实现对良恶性结节的准确鉴别与诊断[4]。此次研究分别通过常规超声、超声弹性成像技术对本院2018年4月-2019年4月本院收治的56例甲状腺微小结节患者展开检查,旨在进一步对甲状腺微小结节良恶性诊断中超声弹性成像的应用价值进行探讨,现报道如下。, 百拇医药(熊良能 鞠敏 鞠海香)
