蓝光照射联合益生菌口服对早产黄疸患儿TCB水平及不良反应的影响(1)

     【摘要】 目的:探讨蓝光照射联合益生菌口服对早产黄疸患儿经皮胆红素(TCB)水平及不良反应的影响。方法:按随机数字表法将2018年1月-2019年2月就诊于本院的60例早产黄疸患儿分为对照组与研究组,各30例。对照组行蓝光照射治疗,研究组行蓝光照射联合益生菌(双歧杆菌三联活菌散)治疗。比较两组临床疗效、胆红素水平及不良反应发生情况。结果:研究组治疗总有效率(90.00%)高于对照组(66.67%),差异有统计学意义(P<0.05)。研究组TCB、血清直接胆红素(DB)、总胆红素(TSB)分别为(196.24±29.96)、(8.14±0.71)、(19.72±1.24)μmol/L均低于对照组的(240.47±30.97)、(10.41±1.18)、(29.70±2.07)μmol/L,差异均有统计学意义(P<0.05)。研究组不良反应发生率(6.67%)低于对照组(30.00%),差异有统计学意义(P<0.05)。结论:对早产黄疸患儿行蓝光照射联合益生菌口服治疗可有效改善黄疸症状,降低胆红素水平,减少不良反应发生,安全性较高。

    [Abstract] Objective: To investigate the effect of blue light irradiation combined with probiotics oral on the level of transcutaneous bilirubin (TCB) and adverse reactions in preterm jaundice children. Method: According to the random number table method, 60 children with premature jaundice admitted to our hospital from January 2018 to February 2019 were divided into the control group and the study group, 30 cases in each group. The control group was treated with blue light irradiation, while the study group was treated with blue light irradiation combined with probiotics (bifidobacterium triple viable powder). The clinical efficacy, bilirubin level and adverse reactions were compared between the two groups. Result: The total effective rate in the study group (90.00%) was higher than that in the control group (66.67%), the difference was statistically significant (P<0.05). TCB, serum direct bilirubin (DB) and total bilirubin (TSB) in the study group were (196.24±29.96), (8.14±0.71), (19.72±1.24)μmol/L respectively lower than those in the control group (240.47±30.97), (10.41±1.18), (29.70±2.07)μmol/L, the differences were statistically significant(P<0.05). The incidence of adverse reactions in the study group (6.67%) was lower than that in the control group (30.00%), the difference was statistically significant(P<0.05). Conclusion: Blue light irradiation combined with probiotics oral in preterm jaundice children can effectively improve jaundice symptoms, reduce bilirubin levels, adverse reactions and have high safety.

    早产儿黄疸发病率约占新生儿黄疸的80%,是由多种因素造成胆红素排泄障碍所致,可引起肝功能损害、全身黄染等,甚至诱发胆红素脑病,影响患儿正常生长发育[1-3]。光疗是临床治疗早产儿黄疸的首选手段,脂溶性胆红素经吸收敏感波长的光线转变为水溶性,从而降低胆红素水平,改善患儿症状[4-5]。但部分患儿治疗后会出现腹泻、皮疹、青铜症等不良反应,需停止治疗。益生菌可促进肠蠕动、降低肠道内β-葡萄糖酸苷酶活性与肠道pH值,促进胆红素排泄并减少肝肠循环,与光疗联合使用,可进一步提高胆红素排泄强度与速度[6-7]。本研究选擇2018年1月-2019年2月就诊于本院的60例早产黄疸患儿,旨在分析蓝光照射联合益生菌口服对早产黄疸患儿TCB水平及不良反应的影响。现报道如下。, 百拇医药(董珊珊 吴淑媛)
