同型半胱氨酸、脂蛋白相关磷脂酶A2水平与颈动脉粥样硬化、狭窄程度的相关性研究(1)

     【摘要】 目的:探討血清同型半胱氨酸(Hcy)、脂蛋白相关磷脂酶A2(Lp-PLA2)水平与颈动脉粥样硬化、狭窄程度的相关性。方法:选择在本院就诊的114例B超检查患不同程度颈动脉粥样硬化患者作为研究组,测量颈动脉血管的内径、内膜、内-中膜厚度(IMT),并判断狭窄程度,采用北美症状性颈动脉内膜剥脱术研究(NASCET)测量狭窄程度,并以远端颈内动脉作为狭窄率的参照,分为轻度狭窄、中度狭窄与重度狭窄。另选择本院同期体检健康者50例作为对照组。分别检测所有研究对象血清Hcy和Lp-PLA2水平。结果:研究组Hcy和Lp-PLA2水平均显著高于对照组(P<0.01);当血清Hcy为17.25 mol/L时预测颈动脉粥样硬化价值最高,灵敏度与特异度分别为82.6%、63.4%;当Lp-PLA2为284.9 g/L时预测颈动脉粥样硬化价值最高,灵敏度与特异度分别为92.5%、71.9%;与轻度狭窄患者比较,中度狭窄和重度狭窄患者Hcy和Lp-PLA2水平均显著升高(P<0.01);与中度狭窄患者比较,重度狭窄患者Hcy和Lp-PLA2水平均显著升高(P<0.01);Hcy和Lp-PLA2与颈动脉狭窄程度均呈正相关(r=0.44、0.61,P<0.01)。结论:Hcy和Lp-PLA2水平与颈动脉粥样硬化关系密切,且可作为判断颈动脉狭窄程度指标。

    [Abstract] Objective: To investigate the relationship between serum homocysteine (Hcy) and lipoprotein- related phospholipase A2 (Lp-PLA2) levels and the degree of carotid atherosclerosis and stenosis. Method: A total of 114 patients with different degrees of carotid atherosclerosis who were examined by B-ultrasound in our hospital were selected as the study group, the internal diameter, intima-media thickness (IMT) and stenosis degree of carotid artery were measured. North American Symptomatic (NASCET) was used to measure the degree of stenosis, and the distal internal carotid artery was used as a reference for stenosis rate. The patients with mild stenosis, moderate stenosis and severe stenosis were divided into mild stenosis group, moderate stenosis group and severe stenosis group. In addition, 50 healthy patients were selected as the control group. The serum Hcy and Lp-PLA2 of all objects were detected respectively. Result: The levels of Hcy and Lp-PLA2 in the study group were significantly higher than those in the control group (P<0.01). When serum Hcy was 17.25 mol/L, the value of predicting carotid atherosclerosis was the highest, the sensitivity and specificity were 82.6% and 63.4%, respectively. When Lp-PLA2 was 284.9 g/L, the value of predicting carotid atherosclerosis was the highest, the sensitivity and specificity were 92.5% and 71.9% , respectively. The levels of Hcy and Lp-PLA2 in moderate stenosis and severe stenosis patients were significantly higher than those in mild stenosis patients (P<0.01), and the levels of Hcy and Lp-PLA2 in severe stenosis patients were significantly higher than those in moderate stenosis patients (P<0.01). There were positive correlation between Hcy and Lp-PLA2 and the degree of carotid artery stenosis (r=0.44, 0.61, P<0.01). Conclusion: The levels of Hcy and Lp-PLA2 are closely related to carotid atherosclerosis and can be used as indicators to judge the degree of carotid stenosis., 百拇医药(宋海涛 李锐锋 刘翠娴 潘成明 胡森安)
