磁共振扩散加权成像和化学位移成像在良恶性椎体骨折早期鉴别诊断中的应用效果及价值(1)

     [摘要] 目的 研究磁共振擴散加权成像(DWI)与化学位移成像(CSI)在良恶性椎体骨折早期鉴别诊断中的意义。 方法 在我院于2015年5月~2016年12月间收治的椎体骨折患者中随机抽取40例为研究对象,对全部患者均进行磁共振扩散加权成像、化学位移成像的检查,对比分析两种方法对于良恶性椎体骨折鉴别斩断的敏感性、特异性、准确性。 结果 定性分析方面:CSI鉴别诊断的准确性、敏感性和特异性分别为65.0%、91.67%、46.43%,经双尾检测发现,差异有统计学意义(P<0.05);DWI鉴别诊断则分别为57.5%、75.0%、50.0%,差异无统计学意义,(P>0.05);定量分析上:CSI鉴别诊断良恶性上发现良恶性患者的IP/OP信号强度比差异有统计学意义(P<0.05);DWI在鉴别诊断良恶性上发现ADC值差异不大,差异无统计学意义(P>0.05)。 结论 综上所述,在良恶性椎体骨折早期鉴别诊断中采用化学位移成像技术的定量分析有助于临床医师获取准确的信息,减少误诊,提高诊断的正确性。

    Application effect and value of diffusion-weighted MR imaging and chemical shift imaging in early differential diagnosis of benign and malignant vertebral fracture

    [Abstract] Objective To research the significance of diffusion-weighted MR imaging (DWI) and chemical shift imaging (CSI) in early differential diagnosis of benign and malignant vertebral fracture. Methods 40 Patients with vertebral fracture who were admitted to our hospital from May 2015 to December 2016 were randomly selected as research objects. All patients were given examinations of diffusion-weighted MR imaging and chemical shift imaging. Sensitivity, specificity and accuracy of two methods in differential diagnosis of benign and malignant vertebral fracture were compared and analyzed. Results By qualitative analysis, accuracy, sensitivity and specificity of differential diagnosis of CSI were 65.0%, 91.67% and 46.43% respectively. Double tails detection showed the difference was statistically significant (P<0.05). Accuracy, sensitivity and specificity of differential diagnosis of DWI were respectively 57.5%, 75.0% and 50.0%. The difference was not statistically significant (P>0.05). By quantitative analysis, IP/PO signal intensity ratios of patients with benign and malignant vertebral fracture of CSI in differential diagnosis were statistically significant (P<0.05). DWI in differential diagnosis of benign and malignant vertebral fracture showed that ADC values had no significant difference and the difference was no statistically significant (P>0.05). Conclusion In conclusion, quantitative analysis of chemical shift imaging in early differential diagnosis of benign and malignant vertebral fracture can help clinicians to acquire accurate information, reduce misdiagnosis and improve accuracy of diagnosis., http://www.100md.com(翟锦添?刘准液)
