H—FABP、hs—cTnI和NT—proBNP检测在慢性心力衰竭诊断和预后评估中的应用(1)
[摘要] 目的 分析檢测心型脂肪酸结合蛋白(H-FABP)、高敏肌钙蛋白T(hs-cTnI)、氨基末端B型钠尿肽原(NT-proBNP)的表达水平对于慢性心力衰竭诊断与预后评估的应用价值。 方法 选取本院2013年3月～2014年3月间收治的89例慢性心力衰竭(CHF)患者作为观察对象纳入CHF组，并以同期在本院接受健康体检的80例健康受检者纳入对照组，检测全部入选者的H-FABP、hs-cTnI、NT-proBNP的表达水平，根据研究分组进行数据对比分析。 结果 CHF组H-FABP、hs-cTnI、NT-proBNP表达水平均显著高于对照组，P<0.05；Ⅲ～Ⅳ组H-FABP、hs-cTnI、NT-proBNP表达水平均显著高于Ⅰ～Ⅱ组，P<0.05；CHF组不良预后发生率为31.46%，预后理想组H-FABP、hs-cTnI、NT-proBNP表达水平均显著低于预后不良组，P<0.05；H-FABP、hs-cTnI、NT-proBNP与NYHA分级、预后情况间均呈显著高度正相关，P<0.05；hs-cTnI、NT-proBNP、H-FABP对于预后评估具有诊断价值，P<0.05。 结论 H-FABP、hs-cTnI和NT-proBNP检测可为慢性心力衰竭的诊断及预后评估提供数据化参考，从而提高了临床诊断与预后评估的准确性，值得临床广泛应用。

[Abstract] Objective To analyze the application value of heart type fatty acid binding protein (H-FABP)，Gao Min (hs-cTnI)，troponin T N-terminal B type natriuretic peptide (NT-proBNP) value of the expression in the diagnosis of chronic heart failure and prognosis assessment. Methods A total of 89 cases of chronic heart failure (CHF) patients in our hospital from March 2013 to March 2014 were selected as the observation object，and they were divided into the CHF group.During the same period，80 cases of healthy subjects in our hospital for health examination were selected as the control group.The expression level of all subjects，H-FABP detection of hs-cTnI， NT-proBNP of the two groups were detected.The data was analyzed. Results The H-FABP，hs-cTnI，NT-proBNP expression levels in CHF group were significantly higher than those in control group，P<0.05.The H-FABP，hs-cTnI，NT-proBNP expression levels in III-IV group were significantly higher than those in I-II group，P<0.05.The occurrence rate of adverse prognosis in CHF group was 31.46%.The H-FABP，hs-cTnI，NT-proBNP expression levels of prognosis of ideal group were significantly lower than those of the poor prognosis group，P<0.05.H-FABP，hs-cTnI，NT-proBNP and NYHA grading and prognosis were significantly positively correlated，P<0.05.the hs-cTnI，NT-proBNP，H-FABP had a high diagnostic value for evaluating prognosis of P<0.05. Conclusion H-FABP，hs-cTnI and NT-proBNP can provide the data reference for the diagnosis and prognosis evaluation of chronic heart failure，so as to improve the accuracy of clinical diagnosis and prognosis evaluation.It is worthy of clinical application.
