Translational Medicine——From Bedside to Bench

Despite unprecedented opportunities in basic and clinical research, the gap between them
has widened. To bridge the gap between these disciplines, a novel conception,“translational medicine” emerged. It can be defined as a discipline in which basic
scientists provide clinicians with new tools for use in patients and for assessment of their
impact, and clinical researchers make novel observations about the nature and
progression of disease that often stimulate basic investigations.
Translational research has been proven to be a powerful process that drives the clinical
research engine since its emergence. It helps translating basic science advances to
improved patient care. Discovery of novel biomarkers for some given diseases is a
prerequisite content of translational medicine, which may promote the development of
new drugs effectively. In our practice, 4 novel HBV-related genes, successfully cloned by
our group, have been demonstrated elevated expression in the canceration group
compared with non-canceration group. Combinational serum screening or even single
protein along can be served as early predictor and prognostic marker for HCC, suggesting
the promising prospect of clinical application of these novel genes. Assay kit for predicting
HCC based on these genes is ongoing.
So far, translational medicine has rather been a linear concept rooted in traditional
approaches to provide therapies for diseases (from bench to bedside), while
patient-oriented research that involves understanding the underlying cause of disease
and its treatments (from bedside to bench) is often regrettably neglected. In practice,translational medicine should be regarded as a two-way road: Bench to Bedside and
Bedside to Bench. Further comprehension of pathogenesis of diseases may trigger new
therapeutic strategy and should be paid more attention. One of our clinical studies
published on Nature Clin Pract Oncol (2005) can be served as a paradigm of bedside to
bench, which is the first Chinese clinical study published on the journal of Nature series.
Most importantly, this study is recommended as CME (continuing medical education) for

The conception of comorbidity was first put forward by Feinstein in 1970s, it refers the
presence of coexisting or additional diseases with reference to an initial diagnosis or with
reference to the index condition that is the subject of study. There is an increased
prevalence of psychiatric disorders in clinic patients with some FGIDs (40%-60%). When
the patients who suffer from psychiatric disorders have the complains characterized by
somatization disorders, their symptoms would be very difficult to get improved without
proper pharmacologic treatment. Traditionally, the gastroenterologist focus on the GUT,but the BRAIN could not be neglected. Knowledge about the psychosocial field of FGIDs ......
