Clinical diagnosis and Dynamically analysis of SAT

[Abstract] Objictive:To deepen our understanding of subacute thyroiditis(SAT) by analyzing clinical material.Method:145 cases With SAT were analyzed based on clinical and nuclear medicine examination datA、7 cases were followed up for ten years.27 cases were dynamically observed with RAIU and r imaging for four months.Results:All of the 145 SAT cases showed abnormal in r imaging,one side of thyroid in 11.8% cases and both sides in 88.2% were found with abnormalities respectively.The cases with RAIU above 5% showed one side of thyroid abnormal,while 97.2% of both sides abnormal cases showed lower RAIU below 2%. Mismatch of thyroid sign and r imaging results happened in 35.1% of cases.TT3、TT4 were found increased or upper values in 60% one side and 94.3% both sides of thyroid abnormalities.90% cases heve showed the separating phenomenon beteen RAIU and thyroid hormone,which usually happened during the first month of disease.Positive rate of TgAB and TmAB was 31.3%.The results of thyroid scan and RAIU returned to normal in 44.4% and 13.3% after one month therapy,while in 88.9% and 100% after four months therapy. In the 7 cases with long term following up, r scan was no change further,RAIU kept lower value or below normal,TSH in most cases was upper value or above normal,TgAB and TmAB remained positive or turn to positive, differ from diagnosis of classical SAT.Conclusion:Combination of cases history,symptoms,sign and nuclear medicine examination assist diagnosis of SAT,besides isuseful for following up of SAT.
