HBV-ACLF患者肝脏B7-H6的表达水平与肝损伤的相关性(1)

     【摘要】 目的 探討HBV相关慢加急性肝衰竭(HBV-ACLF)患者肝组织自然杀伤细胞天然细胞毒性受体NKp30特异性配体B7同源物6(B7-H6)的表达水平与肝损伤的相关性。方法 收集HBV-ACLF患者肝移植术中切除的肝组织(HBV-ACLF组)及CHB患者(CHB组)肝穿刺标本各18例,并以5例来源于肝脏血管瘤切除标本旁的正常肝组织为正常对照组。利用免疫组织化学染色检测3组肝组织中B7-H6的表达水平。分析肝组织B7-H6表达水平及与肝损伤程度(总胆红素水平)的相关性。结果 B7-H6主要表达在肝细胞,而且多在肝小炎症反应强烈区域内。定量分析显示,HBV-ACLF组肝组织B7-H6表达的平均相对密度(127.02±40.25)高于轻度CHB组(22.08±10.82)和正常对照组(14.26±5.67)(P均< 0.05),轻度CHB组与正常对照组比较差异无统计学意义(P > 0.05)。HBV-ACLF患者肝脏B7-H6水平与血清总胆红素水平呈正相关(rs = 0.604,P < 0.05)。结论 肝脏B7-H6高表达可能是NKp30/B7-H6被激活导致HBV-ACLF免疫性肝损伤加剧,肝细胞大量凋亡和坏死的重要因素。

    Correlation between the expression level of hepatic B7-H6 and liver injury in HBV-ACLF patients

    【Abstract】 Objective To investigate the correlation between the expression level of hepatic B7 homologue 6 (B7-H6) and liver injury in patients with hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF). Method Needle biopsy liver tissues and resected liver tissue samples during liver transplantation were obtained from patients with mild chronic hepatitis B (CHB) and HBV-ACLF patients, respectively. Normal liver tissues from 5 cases of hemangioma were utilized as normal controls. The expression levels of hepatic B7-H6 were quantitatively measured by immunohistochemical staining. The correlation between the expression of B7-H6 and the degree of liver injury (total bilirubin level) was analyzed. Results B7-H6 was mainly expressed in liver cells, especially in the area with intense hepatic inflammatory responses. Quantitative analysis demonstrated that the average relative density of B7-H6 expression in the HBV-ACLF group was (127.02±40.25)/lpf, significantly higher compared with (22.08±10.82)/lpf and (14.26±5.67)/lpf in the mild CHB and normal control groups (both P < 0.05). No statistical significance was noted between the mild CHB and normal control groups (P > 0.05). In the HBV-ACLF patients, the B7-H6 expression level was positively correlated with the serum total bilirubin level (rs = 0.604, P < 0.05). Conclusion High expression of hepatic B7-H6 is probably a critical factor leading to aggravation of immunity-mediated liver injury induced by NKp30/B7-H6 activation, the apoptosis and necrosis of a large quantity of liver cells in HBV-ACLF patients., 百拇医药(袁青?杨小安?兰天云?邹勇)
