空肠插管造口术在消化外科肠内营养中的应用(1)

     摘 要 目的:探讨空肠插管造口术在消化外科肠内营养中的应用效果。方法:选取90例消化外科肠内营养的患者,按随机数字表法将患者分为对照组和观察组各45例,分别采用鼻饲法和空肠插管造口术进行肠内营养。随访8周,比较两组患者的营养指标、住院时间、排气时间及并发症发生情况。结果:两组患者营养后体质指数、血清白蛋白、转铁蛋白、血清总蛋白均较营养前明显升高,观察组并发症发生率明显低于对照组(P<0.05)。结论:空肠插管造口术应用于消化外科肠内营养,临床效果与鼻饲法肠内营养相同,但空肠插管造口术能有效降低并发症的发生率。

    Application of jejunum intubation and ostomy in enteral nutrition in digestive surgery*

    ABSTRACT Objective: To explore the effect of jejunal intubation ostomy on enteral nutrition in digestive surgery. Methods: Ninety patients with enteral nutrition in digestive surgery were selected and divided into a control group (nasal feeding) and an observation group (jejunal cannulation) with 45 cases each based on a random number table. The nutritional indicators, hospitalization time, exhaust time and complications were compared between the two groups after 8 weeks follow-up. Results: The body mass index, the levels of serum albumin, transferrin and total serum protein in the two groups were significantly higher after nutrition than before and the incidence of complications was significantly lower in the observation group than the control group (6.67% vs 22.22%, P<0.05). Conclusion: The clinical effect of jejunal intubation ostomy applied to enteral nutrition in digestive surgery is the same as that of nasal feeding, but jejunal intubation ostomy can effectively reduce the incidence of complications.

    消化外科手術后,需要进行营养支持,促进患者的恢复。空肠插管造口术、经鼻腔插管等均为临床常用的肠内营养方法[1]。空肠插管造口术的目的是帮助肠道减压,进行营养灌注。空肠造口肠内营养可以改善患者术后的营养状况,促进患者康复[2-3]。

    1 资料和方法

    1.1 一般资料

    选取2017年12月—2018年12月在深圳市萨米医疗中心行消化外科手术后肠内营养的患者90例,按照随机数字表法将患者分为对照组和观察组各45例。其中对照组男性26例,女性19例;年龄46~76岁,平均年龄(62.7±5.5)岁;胰头癌9例,壶腹部癌10例,胃癌26例。观察组男性27例,女性18例;年龄45~75岁,平均年龄(62.3±5.7)岁;胰头癌10例,壶腹部癌11例,胃癌24例。两组患者一般资料比较差异无统计学意义(P>0.05),具有可比性。本次研究获得医院伦理委员会批准。, 百拇医药(柳长明 崔国利 赵广阳 程开)
