病案首页书写缺陷分析及解决对策(1)
摘要：目的 调查病案首页书写存在的问题，加强病案质量管理，提高病案首页数据的准确性。方法 对某院2018年1月～6月病案首页填写情况进行调查，统计缺陷项目及存在的问题。结果 终末质控归档病案首页48249份，项目漏填率为2.40%;错填率整体较高，死亡与尸检错填率最高，为77.20%，其次为损伤中毒的外部原因(46.00%)、药物过敏(31.89%)、手术切口愈合等级(26.18%)等。每份病例首页均存在填写问题，且问题形式多种多样。结论 医疗机构应加强病案质量管理，医务人员应规范填写首页，降低人为因素造成的错填率，提高病案首页信息质量。

Abstract：Objective To investigate the problems existing in the first page of the medical record， strengthen the quality management of the medical record， and improve the accuracy of the data on the front page of the medical record.Methods Investigate the filling of the first page of the medical record from January to June 2018 in a hospital， and count the defective items and existing problems.Results The final quality control archived medical records were 48，249 on the first page， and the project leakage rate was 2.40%. The overall error rate was higher， and the death and autopsy rate was 77.20%， followed by external causes of damage poisoning (46.00%). Drug allergy (31.89%)， surgical incision healing grade (26.18%). There is a question of filling in the front page of each case， and the form of the problem is various.Conclusion Medical institutions should strengthen the quality management of medical records. Medical staff should standardize the homepage， reduce the rate of incorrect filling caused by human factors， and improve the quality of medical records.

病案是具有法律文书性质的特种档案，而病案首页是病案浓缩的精华，它涵括了患者的基本情况、出院诊断、手术、抢救、费用等信息。目前，随着医疗信息化的普及，病案首页信息的再利用价值越来越高，智慧医院以“病案为核心”具有辅助临床治疗的信息系统数据全部采集于病案首页、医保定额支付标准制定、疾病诊断相关分组(DRGs)的分组依据也源自于病案首页、卫生管理机构对医院质量进行综合评价的数据依据同样来源于病案首页[1-3]。所以加强病案管理，提高首页信息的准确率是当务之急。现对某院病案首页填写质量进行分析，并提出改进方法。
1材料与方法
1.1病案资料 选择某院 2018年1月～6月48249份出院病案进行归档病案质控检查。收集病案首页资料，具体，病案首页具体信息包括基本信息类、诊断信息类、医护信息类、手术信息类、编码信息类及其他信息类等。

1.2方法 根据卫计委发布的《住院病案首页数据填写质量规范(暂行)和住院病案首页数据质量管理与控制指标(2016版)24号》和安徽省下发的《住院病历质量评估标准》对病案首页进行终末质控，未按要求填写、格式无效、违反基本逻辑的视作错填项;必填项及应填但未填的视作漏填项[4，5]。
2结果
质控检查发现项目填写缺项漏填率较低，2018年1～6月某院出院病案首页除基本信息类填写率(97.62%)及其他信息类填写率(56.36%)外，其余信息类填写率均为100.00%，见表1。但病案首页错填率高，首页项目间相互矛盾、首页项目与病程记录、检查报告、医嘱等不一致情况较常见，具体错填比率及原因见表2。, 百拇医药(张中花)
