经尿道膀胱黏膜电凝术联合甲醛溶液灌注治疗出血性放射性膀胱炎的临床效果(1)

     摘要:目的 观察放射性膀胱炎患者接受经尿道膀胱黏膜电凝术联合2.5%甲醛溶液膀胱灌注治疗的临床效果及安全性。方法 选择2012年6月~2016年6月我院收治的21 例放射性膀胱炎患者,均行经尿道膀胱黏膜电凝术联合2.5%甲醛溶液膀胱灌注治疗,记录患者术前、术后1个月、术后3个月血红蛋白水平,比较患者术前及术后1个月血尿分级水平,随访2年内患者并发症情况。结果 患者术前、术后1个月、术后3个月血红蛋白水平分别为(65.10±15.43)g/L、(93.24±12.01)g/L、(106.62±11.65)g/L,差异均有统计学意义(P<0.05);患者术前及术后1个月血尿分级比较,差异具有统计学意义(P<0.05);所有患者随访2年中均未见膀胱挛缩情况,无再出血患者。21例患者中2例患者术后出现尿频、尿急症状,给予对症治疗后,患者症状明显改善。结论 经尿道膀胱黏膜电凝术联合2.5%甲醛溶液膀胱灌注治療放射性膀胱炎不良反应较少,安全性较高,可有效改善患者血尿情况。

    Abstract:Objective To observe the clinical efficacy and safety of transurethral bladder mucosal electrocoagulation combined with 2.5% formaldehyde solution for bladder perfusion in patients with radiation cystitis.Methods 21 patients with radiation cystitis admitted to our hospital from June 2012 to June 2016 were treated with transurethral bladder mucosal electrocoagulation combined with 2.5% formaldehyde solution for bladder perfusion. The patients were recorded before and 1 month after surgery. The hemoglobin level was 3 months after operation, and the hematuria grading level before and 1 month after surgery was compared. The complications of the patients were followed up for 2 years.Results The hemoglobin levels of patients before operation, 1 month after operation and 3 months after operation were (65.10±15.43)g/L, (93.24±12.01)g/L, and (106.62±11.65)g/L, respectively,the difference was statistically significant (P<0.05), and there was significant difference in hematuria grading before operation and one month after operation (P<0.05). No bladder contracture was found in all patients during the follow-up of 2 years, and there was no rebleeding. Of the 21 patients, 2 had frequent urination and acute urinary symptoms, and the symptoms of the patients improved obviously after treatment with symptomatic treatment.Conclusion Transurethral bladder mucosal electrocoagulation combined with 2.5% formaldehyde solution for intravesical instillation of radioactive cystitis has fewer adverse reactions and higher safety, which can effectively improve hematuria.
