磁共振脑灌注成像在先天性血管狭窄脑组织血供与临床评估中的价值(1)

     [摘要] 目的 應用磁共振灌注成像技术评价先天性血管狭窄患者的脑血流动力学情况。 方法 选取2016年6月~2018年12月期间就诊我院通过DSA或CTA已确诊存在先天性血管狭窄患者48例,选择感兴趣区通过Functool软件进行后处理得到rCBV、rCBF、rMTT及rTTP等相关参数数值。比较存在临床症状患者与无临床症状患者灌注上差异及存在临床症状患者不同区域的灌注情况。 结果 比较有临床症状患者与无临床症状患者灌注rCBV、rCBF、rMTT及rTTP差异,rCBF数值减低、而rMTT和rTTP数值呈现出延长(P<0.05),而两组间rCBV值比较差异无统计学意义(P>0.05);先天性血管狭窄患者狭窄区供血rCBF及rCBV值减低、rMTT及rTTP值出现延长,差异有统计学意义(P<0.05)。 结论 磁共振脑灌注功能成像技术可以有效评估先天性血管狭窄脑组织血供情况,磁共振脑灌注成像在先天性血管狭窄脑组织血供与临床评估中具有重要价值。

    Value of magnetic resonance cerebral perfusion imaging in brain tissue blood supply and clinical evaluation of congenital vascular stenosis

    [Abstract] Objective To evaluate the cerebral hemodynamics of patients with congenital vascular stenosis using magnetic resonance perfusion imaging. Methods 48 patients with congenital vascular stenosis who were diagnosed with DSA or CTA in our hospital from June 2016 to December 2018 were selected. The region of interest was selected and post-processing with Functool software was performed to obtain the relevant parameter values such as rCBV,rCBF, rMTT, and rTTP. The differences in perfusion between patients with clinical symptoms and those without clinical symptoms,and the perfusion in different areas of patients with clinical symptoms were compared. Results The differences in perfusion rCBV, rCBF, rMTT and rTTP between patients with clinical symptoms and those without clinical symptoms were compared. The rCBF values were decreased, while the rMTT and rTTP values were prolonged(P<0.05). There was no statistical difference in rCBV between the two groups(P>0.05). The rCBF and rCBV values were decreased, and rMTT and rTTP values were prolonged in patients with congenital vascular stenosis,with statistically significant difference(P<0.05). Conclusion Magnetic resonance perfusion functional imaging can effectively evaluate the blood supply of cerebral tissues with congenital cerebrovascular stenosis. Magnetic resonance perfusion imaging has important value in the brain tissue blood supply and clinical evaluation of congenital vascular stenosis.
