改善执业药师在医院现行政策机制下的工作职能(1)
[摘要]执业药师作为药学人才体系中重要的组成部分，其主要工作职能是促进药学服务的规范化和药品质量的保障工作。在医院内的执业药师群体由于多种原因，仅从事着医院药学工作：药品调剂、用药指导和一般的药品质量保障和服务工作。基于医院内执业药师队伍的发展和工作现状，本文结合“医院药学工作要建立以病人为中心的药学管理工作模式”要求，探讨执业药师应适时转变职能，履行合理用药职责，参与临床药学服务，努力保障医疗质量，提高安全防范。在医院现行制度下，充分落实执业药师在质控管理、抗生素分级管理、高危药品管理、合理用药监管平台建设、中药安全使用、药品使用信息化进程及不良反应监测等具体工作中的职责。

[Abstract]Licensed pharmacist is an important part of pharmaceutical human resource system，whose key job functions are the improvement of the standardized management of pharmaceutical services and the quality assurance of pharmaceutical products.Due to incomplete policies and some subjective reasons，licensed pharmacists in hospitals only work on drug dispensing，medication guiding and the routine quality assurance and services of drugs in the hospital pharmacy.Based on the development and present situation of licensed pharmacists working in hospitals，considering the requirement that "a patient-centered pharmaceutical management mode should be established in the hospital pharmacy"，this study discussed the issues that licensed pharmacists should timely change their job functions，perform the duties of rational medication，participate in clinical pharmacy services and try to improve the quality of medical care and safety precaution.Fully implement the specific responsibilities of licensed pharmacists under the existing hospital system，which include strengthening quality control and management，graded management of antibiotics，high-risk drug management，the construction of monitoring platform of rational medication，the safe medication of traditional Chinese medicine，the informationization process of medication，the monitoring of adverse reactions.

执业(中)药师是经考试取得《执业药师资格证书》，并经注册登记，在药品生产、经营、使用单位中执业的药学技术人员。医疗机构作为药品的使用单位，尤其需要有执业资格的药学人员积极承担起用药规范化管理，保障药品质量和临床药学技术服务工作。现阶段，医院内的执业药师人群日益扩大，但执业药师仍局限于传统的工作模式[1]，未能充分发挥其指导合理用药和药品质量管理方面的作用，这主要由法律、条文不明确、医院对药学岗位缺乏足够重视以及执业药师本身专业能力不足所致[2]。随着医药卫生体制改革的进一步推进，医院药学部门面临诸多新政策、条例的颁布实施，药品使用的安全性、质量性和经济性越来越受到重视[3]，在药学服务重心向“以人为本”转变的过程中，明确执业药师执业地位、权利、工作职能，能够极大地发挥执业药师作用，促进合理用药和药品质量管理。基于执业药师在医院中有别于其他药学人员的特殊地位，注册准入制度决定了其职责范围更广，专业能力要求更高。在实施药品质量和使用的监督、管理过程中，如何实现执业药师突破现有职责，转变服务观念，充分履行处方的审核及监督调配，提供用药咨询与信息，指导合理用药，开展治疗药物的监测及药品疗效的评价等工作职责，需要院部、执业药师自身的共同努力。
1医疗机构内执业药师、药师与临床药师的共性与现状
1.1共性
我国实行执业药师资格准入制度，而医院药学人员属于取得卫生专业技术资格职称的药学技术人员。随着医院药学重心由“保障供应”向“技术服务”的深层次转变，临床药师的配备人数得到了极大地发展，临床药学工作越来越得到认可和重视。在医院药学人才体系中，执业药师、药师与临床药师三者共同的职责特点均是以保障药品质量为基本准则，并促进合理用药，确保用药安全、有效。 (苏景红 陆卫东)
