3D腹腔镜和2D腹腔镜下腹膜外途径前列腺癌根治术的优劣差异(1)
【摘要】 目的：觀察3D腹腔镜与2D腹腔镜在经腹膜外途径前列腺癌根治术(laparoscopic radical prostatectomy，LRP)中差异。方法：选取2015年2月-2017年8月本院收治的局限性前列腺癌(prostate cancer，PCa)患者42例。按照随机数字表法将其分为3D组(n=21)和2D组(n=21)。两组均行LRP术，3D组采用3D腹腔镜，2D组采用2D腹腔镜。术后随访3个月，比较两组围手术期结果、并发症、功能恢复情况及肿瘤结局指标。结果：3D组手术时间、术中出血量、尿道重建时间均少于2D组，比较差异均有统计学意义(P<0.05)，两组术后留置导尿管时间和住院时间比较，差异均无统计学意义(P>0.05)；3D组术后尿失禁发生率低于2D组，尿控率高于2D组，比较差异均有统计学意义(P<0.05)，而两组术后尿漏、尿道狭窄发生率比较差异均无统计学意义(P>0.05)；3D组性神经保留者术后勃起功能恢复率高于2D组(P<0.05)；两组肿瘤结局指标比较，差异均无统计学意义(P>0.05)。结论：与2D组相比，3D组的手术时间更短，术中尿道重建时间更短，术中出血量更少，并能改善患者术后尿控及性功能的恢复，但不能改善肿瘤结局预后。

【Abstract】 Objective：To observe the difference between 3D and 2D laparoscopic radical prostatectomy(LRP) for prostate cancer by retroperitoneal approach.Method：A total of 42 patients with prostate cancer(PCa) admitted in our hospital from February 2015 to August 2017 were selected.According to the random number table method，they were divided into 3D group(n=21) and 2D group(n=21).They underwent LRP operation，3D group underwent 3D laparoscopy and 2D group underwent 2D laparoscopy.The follow-up period was 3 months，and the perioperative outcomes，complications，functional recovery and tumor outcome were compared between two groups.Result：The operative time，volume of intraoperafive blood loss，urethral reconstruction time in 3D group were less than those of 2D group，the differences were statistically significant(P<0.05).The indwelling catheter time and hospitalization time in two groups were compared，the differences were not statistically significant(P>0.05).The incidence of urinary incontinence after operation in 3D group was lower than that of 2D group，and the rate of urinary control was higher than that of 2D group，the differences were statistically significant(P<0.05)，and the incidence of urinary leakage and urethral stricture in two groups were compared，the differences were not statistically significant(P>0.05).The recovery rate of erectile function after operation of sex nerve reservations in 3D group was higher than that of 2D group(P<0.05)，and the tumor outcome indicators in two groups were compared，the differences were not statistically significant(P>0.05).Conclusion：Compared with the 2D group，the operation time of the 3D group is shorter，the reconstruction time of the urethra is shorter，the intraoperative blood loss is less，and the recovery of postoperative urinary control and sexual function could be improved，but the prognosis of tumor outcome could not be improved.
